摘　　要：目的探索PDCA循环法在改善丙戊酸血药浓度监测室内质控达标情况、节约检测成本的应用效果。方法通过对2019年本实验室丙戊酸失控质控的回顾性分析,应用PDCA循环法确定关键失控因素,采取针对性整改措施,比较实施PDCA循环干预前后质控分布情况、关键失控因素导致的失控频数及单个试剂盒的平均完成样本量等。结果实施PDCA循环干预后,质控的分布情况较循环前得到较大改善,L1、L2、L3水平质控在±2 S、±3 S区域内的分布明显增加(P<0.05);关键失控因素导致的失控频数明显降低;单个试剂盒的平均完成样本量也显著提升(79.5±8.58 vs.95.5±3.70,P=0.014)。结论PDCA循环法在提高实验室检测水平、试剂盒利用率及节约检测成本方面成效显著。Objective To explore the application effect of PDCA cycle in improving internal quality control of valproic therapeutic drug monitoring and saving the cost of detection.Methods With retrospective analysis of valproic out-of-control quality controls in 2019,the PDCA cycle is applied to determine the key out-of-control factors;take targeted rectification measures,compare distribution of quality control,frequency of out-of-control and the average completion samples of a single kit before and after the PDCA cycle.Results After the implementation of PDCA cycle,the distribution of internal quality control was greatly improved,the distributions of L1,L2,L3 quality control in±2 S and±3 S were significantly more than before the PDCA cycle(P<0.05);the frequency of out-of-control caused by the key out-of-control factors was obviously reduced;the average completion samples of a single kit also increased remarkably(79.5±8.58 vs.95.5±3.70,P=0.014).Conclusion PDCA cycle has a significant effect on the management of the internal quality control,which can effectively improve the laboratory detection capability and save testing costs.
