摘　　要：目的:编制中老年髋关节置换术后病人护理门诊需求量表,并检验其信效度。方法:以马斯洛需求层次理论和拉扎勒斯的压力应对模式为理论基础,采用文献回顾、半结构式访谈法形成量表的条目池,经专家函询和预调查对量表条目进行修订和筛选,形成正式量表。采用便利抽样法,于2021年3月—2021年7月选取河南省3所三级甲等医院骨科门诊就诊的302例中老年髋关节置换术后病人进行调查,并对其进行项目分析、信效度检验。结果:量表包含32个条目,探索性因子分析提取5个公因子(躯体舒适需求、康复指导需求、专科护理需求、健康指导需求、社会心理需求),累计方差贡献率为71.050%。量表内容效度指数为0.86,各条目内容效度指数为0.82~1.00,量表平均内容效度指数为0.98,量表Cronbach′s α系数为0.946,Guttman折半信度为0.905,重测信度系数为0.922。结论:编制的中老年髋关节置换术后病人护理门诊需求量表具有良好的信效度,可用于评估病人对护理门诊的需求情况。Objective:To develop a nursing outpatient demand scale for middle-aged and elderly patients after total hip arthroplasty(THA)and to test its reliability and validity. Method:Based on Maslow′s hierarchy of needs and the Stress and Coping Model(Lazarus and Folkman),the item pool of the scale was formed by literature review and semi-structured interview method,then revised and screened through expert correspondence and pre-investigation to form a formal scale. A total of 302 middle-aged and elderly postoperative patients with THA from March to July 2021 in the orthopedic outpatient clinic of 3 tertiary grade A hospitals in Henan province were selected using the convenience sampling method. The scale was used to investigate the patients,the item analysis was conducted,and the reliability and validity were tested. Results:The scale contained 32 items and 5 common factors(physical comfort needs,rehabilitation guidance needs,specialized nursing needs,health guidance needs,and psychosocial needs)extracted by exploratory factor analysis,and the cumulative variance contribution rate was 71. 050%. The content validity index of the scale was 0. 86,the content validity index of each item was 0. 82-1. 00,the average content validity index of the scale was 0. 98,the Cronbach′s α coefficient of the scale was 0. 946,the half-way reliability was 0. 905,and the test-retest reliability was 0. 922. Conclusion:The nursing outpatient demand scale for middleaged and elderly patients after THA has good reliability and validity,which could be used to evaluate patients′ demand for nursing outpatient service.
