颅脑损伤患者血清可溶性髓系细胞触发受体-1浓度检测的临床意义

摘  要:目的分析血清可溶性髓系细胞触发受体-1(sTREM-1)浓度与颅脑损伤(TBI)创伤程度和3个月预后不良的相关性。方法招募102例TBI患者及102例健康者,测定血清sTREM-1浓度。分析患者血清sTREM-1浓度与格拉斯哥昏迷(GCS)评分和鹿特丹CT评分的相关性,及其与患者外伤后3个月预后不良(扩展格拉斯哥预后评分1~4分)的相关性及预后预测价值。结果TBI患者血清sTREM-1浓度较对照组升高(P<0.01)。血清sTREM-1浓度与患者GCS评分呈负相关性(P<0.01),与患者鹿特丹CT评分呈正相关性(P<0.01),是患者外伤后3个月预后不良的独立危险因素(P<0.05)。血清sTREM-1浓度的预后预测效能与GCS评分及鹿特丹CT评分比较,差异无统计学意义(P>0.05),且血清sTREM-1浓度检测联合GCS评分和鹿特丹CT评分能提高预后预测效能(P<0.05)。结论TBI后高血清sTREM-1浓度与伤情程度密切相关,可独立预测TBI患者不良预后。Objective To investigate relationships between serum soluble triggering receptor expressed on myeloid cells-1(sTREM-1)concentrations and trauma severity in addition to 3-month poor prognosis of traumatic brain injury(TBI).Methods A total of 102 TBI patients and 102 healthy subjects were recruited and serum sTREM-1 concentrations were measured.Change of serum sTREM-1 concentrations of patients and their relations to Glasgow Coma Scale(GCS)scores and Rotterdam CT scores were assessed.The relationship between serum sTREM-1 concentrations and post-traumatic 3-month poor outcome(Extended Glasgow outcome scale score 1-4)of patients as well as their prognostic predictive value were analyzed.Results TBI patients had significantly higher serum sTREM-1 concentrations than controls(P<0.01).Serum sTREM-1 concentrations were significantly correlated with GCS scores inversely(P<0.01)and with Rotterdam CT scores positively(P<0.01),as well as emerged as an independent predictor of 3-month poor outcome(P<0.05).When its prognostic predictive eficiency was compared to those of GCS scores and Rotterdam CT scores,the diferences were not statistically significant(both P>0.05).Serum sTREM-1 concentrations substantially improved prognostic predictive efficiencies of GCS scores and Rotterdam CT scores(both P<0.05).Conclusion Rising serum sTREM-1 concentrations after TBI are closely correlated with trauma severity and can independently predict poor prognosis of patients with TBI.
