摘 要:目的探讨高分辨率CT(HRCT)在尘肺病小阴影聚集和大阴影诊断中的价值。方法选取具有完整胸片和HRCT影像资料的Ⅱ~Ⅲ期尘肺患者123例,对比分析两种检查方法对尘肺小阴影聚集和大阴影的检出率、小阴影总体密集度、尘肺病分期诊断结果。结果两种检查方法小阴影聚集和大阴影检出率相同105例,HRCT高于胸片15例,低于胸片3例;小阴影总体密集度相同111例,HRCT高于胸片7例,低于胸片5例;诊断期别相同114例,HRCT高于胸片8例,低于胸片1例,对两种检查结果进行一致性分析,Kappa值>0.75,一致性好,差异无统计学意义(P>0.05)。结论HRCT更能客观准确显示尘肺小阴影聚集和大阴影,从而有助于Ⅱ~Ⅲ期尘肺的分期诊断。Objective To explore the value of high resolution CT(HRCT)in the diagnosis of small shadow aggregation and large shadow in pneumoconiosis.Methods A total of 123 cases of pneumoconiosis in stage Ⅱ and Ⅲ with complete chest radiography and HRCT images were selected,and the detection rates of small shadow aggregation and large shadow of pneumoconiosis,the overall density of small shadow and the staging diagnosis results of pneumoconiosis by the two examinations were compared and analyzed.ResultsThe detection rates of small shadow accumulation and large shadow by the two methods were the same in 105 cases,and HRCT was higher than chest radiography in 15 cases and lower than chest radiography in 3 cases.The overall density of small shadows was the same in Ⅲ cases,and HRCT was higher than chest radiography in 7 cases and lower than chest radiography in 5 cases.The diagnostic stages were the same in 114 cases,HRCT was higher in 8 cases than chest radiography and lower in 1 case.The results of the two examinations showed good consistency with Kappa value>0.75 and no statistical difference(P>0.05).Conclusions HRCT can more objectively and accurately show the smallshadow accumulation and large shadow of pneumoconiosis,thus contributing to the staging diagnosis of stage Ⅱ and Ⅲ pneumoconiosis.
