腺样体肥大儿童变应原检测结果分析

摘　　要：目的分析腺样体肥大儿童血嗜酸性粒细胞和血清过敏原的检测结果。方法回顾性分析2022年4月-2022年8月常州市儿童医院耳鼻咽喉科以腺样体肥大收住入院的患儿181例,根据是否伴有变应性鼻炎(AR)相关症状分为不伴有变应性鼻炎相关症状的AH组(97例)和伴有变应性鼻炎相关症状的AH+AR组(84例)。所有患儿均抽取外周血检测嗜酸性粒细胞计数,并进行过敏原筛查检测。结果AH+AR组中外周血嗜酸性粒细胞计数较AH组增高,差异具有统计学意义(P<0.01)。AH+AR组中变应原检测阳性率为79.76%,其中最常见的变应原为户尘螨(47例)、粉尘螨(46例)、霉菌混合(28例)。单一变应原阳性14例,阳性率16.67%,两种及以上变应原阳性人数53例,阳性率63.10%。结论变态反应的存在可能是造成腺样体增生肥大的因素之一,户尘螨、粉尘满和霉菌混合是主要的变应原。Objective To analyze the clinical characteristics of serum eosinophils and serum aller⁃gens in children with adenoid hypertrophy.Method A total of 181 children with adenoid hypertrophy ad⁃mitted to the Department of otorhinolaryngology of Changzhou Children's Hospital from April 2022 to Au⁃gust 2022 were retrospectively analyzed.According to the presence or absence of allergic rhinitis(AR)re⁃lated symptoms,they were divided into AH group(n=97)without allergic rhinitis related symptoms and AH+AR group(n=84)with allergic rhinitis related symptoms.Peripheral blood samples were collected from all children for eosinophil count and allergen screening.Results The peripheral blood eosinophil count in AH+AR group was higher than that in AH group,and the difference was statistically significant(P<0.01).The positive rate of allergens in AH+AR group was 79.76%,among which the most common al⁃lergens were house dust mite(47 cases),dust mite(46 cases)and mixed mold(28 cases).The positive rate of single allergen was 16.67%in 14 cases,and the positive rate of two or more allergens was 63.10%in 53 cases.Conclusion The presence of allergy may be one of the factors causing adenoid hyperplasia and hypertrophy.House dust mite,dust mite and mold mixture are the main allergens.
