摘  要:目的:观察肃肺和胃汤对反流性胃肺疾病大鼠肺、胃组织蛋白激酶A(PKA)、环腺苷酸(cAMP)、P物质(SP)表达及磷酸化蛋白激酶A(p-PKA)、瞬时受体电位香草酸亚型1(TRPV1)表达的影响,探讨肺胃同治法治疗反流性胃肺疾病的作用机制。方法:30只SD大鼠,按随机数字表法分为正常对照组、模型组、中药组、西药组、中西医结合组,每组6只,除正常对照组外均采用慢性食管内灌注盐酸(含蛋白酶)方法建立反流性胃肺黏膜慢性炎症大鼠模型。中药组和西药组从造模第15天开始分别予肃肺和胃汤流浸膏(6.7 g/kg)和雷尼替丁(27 mg/kg)灌胃,中西医结合组同时给予等剂量肃肺和胃汤流浸膏和雷尼替丁,正常对照组及模型组给予同剂量生理盐水,连续灌胃7 d。取肺、胃组织进行病理形态学观察,免疫组化法测定PKA、cAMP、SP表达,用Western Blot检测p-PKA、TRPV1蛋白的表达水平。结果:各给药组大鼠肺、胃组织炎症病理半定量积分较模型组低。与正常对照组比较,模型组大鼠肺、胃组织PKA、cAMP、SP及TRPV1的蛋白表达量均极显著升高(P<0.01)。与模型组比较,中西医结合组和西药组大鼠肺组织PKA、cAMP、SP蛋白表达量均显著降低(P<0.05或P<0.01),各给药组大鼠胃组织PKA、cAMP和SP蛋白表达量显著降低(P<0.05或P<0.01),各给药组大鼠肺、胃组织p-PKA和TRPV1蛋白表达水平均显著降低(P<0.05或P<0.01)。结论:肺胃同治法治疗反流性胃肺疾病作用机制可能与通过调控PKA依赖的cAMP信号介导,从而调节TRPV1的激活和SP释放有关。Objective:To observe the effects of Sufei Hewei Decoction on the expressions of PKA、cAMP、SP and phosphorylated PKA、TRPV1 in the lung and stomach tissues of rats with reflux gastro-pulmonary disease,and to study mechanism of lung and stomach common therapy on reflux gastro-pulmonary disease.Methods:30 male SD rats were divided into normal control group,model group,Chinese medicine group,Western medicine group,and integrated Chinese and Western medicine group according to the random number table method,each with 6 rats.The model of chronic inflammation of reflux gastric and lung mucosa in rat was established by infusion of hydrochloric acid(including protease).The Chinese medicine group and the Western medicine group were given Sufei Hewei Decoction extract(6.7 g/kg)and Ranitidine(27 mg/kg)by gavage from the 15 th day of model building.The integrated Chinese and Western medicine group was given the same dose of Sufei Hewei Decoction and Ranitidine.Normal control group and model group were given the same dose of normal saline.,all were medicated for 7 days.After the experiment,the lung and stomach tissue samples were extracted for pathological observation,and the expressions of PKA,cAMP,SP were measured by immunohistochemistry,and the expression levels of p-PKA and TRPV1 protein in the tissue were detected by Western Blot.Results:The lung and stomach tissue inflammation pathological semi-quantitative score of each medication group of rats were lower than that of model group.Compared with the normal control group,the protein expressions of PKA,cAMP,SP and TRPV1 in the lung and stomach tissues of the model group were increased significantly(P<0.01).Compared with the model group,the protein expressions of PKA,cAMP and SP in the lung tissues of the rats in the integrated Chinese and Western medicine group and the Western medicine group were significantly reduced(P<0.05 or P<0.01),and PKA and cAMP in the stomach tissues of the rats in each medication group.The protein expressions of PKA and SP were significantly re
