二白膏外用治疗手部角化性湿疹血虚风燥证49例临床观察

摘　　要：目的观察二白膏外用治疗手部角化性湿疹血虚风燥证的临床疗效及安全性。方法收集手部角化性湿疹血虚风燥证患者92例,随机分为治疗组49例和对照组43例。治疗组患者给予二白膏外用,按约每2 cm×1 cm体表面积取用1食指尖的药量均匀外涂在皮损处,配合轻柔按摩2 min,保留30 min后再擦除;对照组给予氧化锌软膏外用,方法同治疗组。两组患者均早晚各1次,每日用药2次,连续用药28天。于治疗前及治疗14、28天时采用标准化Jena手部湿疹评分表(JHS)评分和瘙痒视觉模拟评分法(VAS)进行评价,治疗14天、28天判定皮损症状疗效和瘙痒症状疗效,同时记录受试者的不良反应。结果治疗前两组患者JHS评分比较差异无统计学意义(P>0.05),治疗组瘙痒VAS评分高于对照组(P<0.05)。与本组治疗前比较,两组患者治疗14天及28天JHS评分及瘙痒VAS评分均明显下降(P<0.05),治疗28天JHS评分及瘙痒VAS评分较治疗14天降低(P<0.05)。治疗14天两组患者JHS评分及瘙痒VAS评分比较差异无统计学意义(P>0.05),而治疗28天治疗组JHS及瘙痒VAS评分均低于对照组(P<0.05)。治疗14、28天,治疗组皮损症状总有效率分别为63.27%、97.96%,瘙痒症状总有效率分别为70.21%、95.74%;对照组治疗14、28天皮损症状总有效率为46.51%、83.72%,瘙痒症状总有效率分别为27.78%、58.33%;治疗组均优于对照组(P<0.05或P<0.01)。治疗期间两组均未见明显不良反应。结论二白膏和氧化锌软膏对于手部角化性湿疹血虚风燥证均安全有效,且二白膏优于氧化锌软膏。Objective To investigate the efficacy and safety of external use of Erbai Ointment(二白膏)in the treatment of hand keratotic eczema with blood deficiency and wind dryness syndrome.Methods Ninety-two patients with hand keratotic eczema and blood deficiency and wind dryness syndrome were included and were randomly divided into treatment group(49 cases)and control group(43 cases).The treatment group was given Erbai Ointment for external use,taking one index fingertip ointment on every 2 cm×1 cm body surface area,massage gently for two minutes,and then wiped off after left on for 30 minutes.The control group was given zinc oxide ointment for external use,with the same application method as the treatment group.Patients in both groups were treated twice a day,one time each in the morning and evening,continuously for 28 days.The standardized Jena hand eczema score(JHS)and the visual analog score(VAS)were assessed before treatment,after 14-and 28-day treatment.The efficacy on skin lesion and pruritus were judged after 14-and 28-day treatment.Meanwhile,the adverse reactions of the subjects were recorded.Results There was no significant statistical difference in JHS scores between groups before treatment(P>0.05),while the VAS score of pruritus was significantly higher in the treatment group than in the control group(P<0.05).The JHS score and pruritus VAS score of patients in both groups decreased significantly after 14-and 28-day treatment(P<0.05),and were decreased more after 28-day treatment than 14-day treatment(P<0.05).No significant differences were observed in JHS score and pruritus VAS score between groups after 14-day treatment(P>0.05),while the JHS and VAS scores in the treatment group after 28-day treatment were lower than those in the control group(P<0.05).The total effective rates on skin lesions after 14-and 28-day treatment were 63.27%and 97.96%,respectively,while those on pruritus were 70.21%and 95.74%in the treatment group.In control group,the 14-and 28-day total effective rates were 46.51%and 83.72%on
