摘　　要：嗓音疾病病位在喉,病因繁多,是多因素共同作用导致的,基本病机为喉部津血瘀滞、气机不畅、喉窍不利。以“三因学说”指导病因分类,将其分为外因(外邪侵袭)、内因(脏腑虚损、功能失调)、不内外因(包括嗓音滥用、暴咳久咳、手术或外伤、情志刺激等)。提出病位为主、整体为辅、防治并重的诊治思路,以消除喉部瘀滞、疏利喉部气机、通利喉窍、开喉亮音为主要治则,借鉴“君、臣、佐、使”的配伍思路,制定出针刺为君、推拿为臣、中药内服为佐、发声矫治训练为使的中医综合治疗方案。Voice disorder is located in the larynx,and caused by the combined action of multiple factors.The basic pathogenesis is stagnation of body fluids and blood stasis in the larynx,disordered qi movement,and dysfunction of throat orifices.According to the“theory of three types of disease causes”,the causes of voice disorder are divided into external causes(invasion of external pathogens),internal causes[zang-fu(脏腑)organs depletion,dysfunction],and causes that are neither internal nor externa,such as vocal irregulare use,protracted cough,surgery or trauma,emotional stimulus.It is advocated to put focus on the disease location,treat the whole as a supplement,and attach importance on both prevention and treatment.The main treatment principles are eliminating stagnation and unblocking qi movement in the throat,clearing the throat orifice,and opening the throat to brighten the sound.Adopting the combination idea of medicinals from“chief,deputy,assistant and envoy”,a comprehensive treatment protocol for voice disorder has been made by taking acupuncture as chief,tuina as deputy,oral herbal medicine as assistant and phonation correction training as envoy.
