摘  要:目的探讨孕期重金属暴露及相关元素与早产发生的关系。方法选取2018年10月1日—2019年9月30日期间在上海交通大学医学院附属国际和平妇幼保健院就诊的16~20周孕妇纳入队列,进行血清金属相关元素检测,采用logistic回归分析探讨重金属元素和相关元素水平对早产的影响。结果足月组和早产组母体血清中所检测的14种ETEs均在正常范围内。且在正常值范围内,母体血清中钙(aOR=1.057,95%CI:1.008~1.108,P<0.05)和汞(aOR=3.441,95%CI:1.748~6.776,P<0.05)的含量越高,早产的发生风险也相应增加;而母体血清中镍含量的增高与早产发生风险的降低有关(aOR=0.625,95%CI:0.406~0.960,P<0.05)。结论此结果为孕期相关元素的检测和合理补充提供了依据,同时母体血清镍与早产发生的关系和相关机制有待进一步研究。Objective To explore the relationship between prenatal exposure to heavy metals and related elements and premature delivery.Methods Pregnant women of 16-20 weeks who visited the International Peace Maternity and Child Health Hospital,School of Medicine,Shanghai Jiao Tong University from October 1,2018 to September 30,2019 were included in the cohort,and serum trace elements were tested.logistic regression analysis was used to explore the impact of trace elements on preterm labour.Results The study found that the higher the content of calcium(aOR=1.057,95%CI:1.008-1.108,P=0.021)and mercury(aOR=3.441,95%CI:1.748-6.776,P=0.000)in maternal serum,the higher the risk of premature delivery;the increase of nickel content in maternal serum was related to the reduction of the risk of premature delivery(aOR=0.625,95%CI:0.406-0.960,P=0.032).Conclusion This result provides a basis for the detection and reasonable supplement of trace elements during pregnancy.
