摘  要:目的 探讨实时言语重读训练治疗痉挛型脑瘫儿童言语流利性障碍的临床疗效。方法 选取2020年10月至2021年3月就诊且符合入选标准的44例5~7岁痉挛型脑瘫儿童为研究对象,按随机数表法分为观察组与对照组,每组22例。对照组进行常规言语康复训练,观察组在此基础上进行实时言语重读训练,每组患者共训练10次,每次30分钟,每日至多1次,在3周内完成。在治疗前、后均选择梁峻波改编版《唱歌篇》进行言语流利性评估,比较两组患者在模仿跟读语言任务下语速、停顿、拖延与重复的评估结果,评价其临床疗效。结果 治疗前组间比较,两组患者言语速率、构音速率、停顿次数、停顿时长、拖延次数、拖延时长与重复次数等7个言语流利性功能指标差异均无统计学意义(均为P>0.05);治疗后组间比较,两组患者7个言语流利性功能指标差异有统计学意义(均为P<0.05),观察组分别为2.08±0.15个/秒、2.83±0.18个/秒、18.41±1.62个/秒、12.28±1.36秒、17.59±2.04次、10.37±1.51秒、3.00±1.23次,显著优于对照组(分别为1.60±0.06个/秒、2.56±0.12个/秒、35.23±3.13次、22.89±1.66秒、37.05±3.27次、22.58±1.88秒、5.18±1.74次);治疗后组内比较,观察组患者7个言语流利性功能指标均较治疗前(分别为1.55±0.15个/秒、2.50±0.33个/秒、36.55±3.57次、24.32±3.27秒、35.50±4.15次、20.62±3.24秒、5.82±2.72次)显著改善(均为P<0.05),而对照组与治疗前比较差异均无统计学意义(均为P>0.05)。结论 实时言语重读训练治疗痉挛型脑瘫儿童言语流利性障碍临床疗效显著。Objective To study the clinical effect of real-time accent method on speech fluency disorders in children with spastic cerebral palsy. Methods A total of 44 children aged 5~7 years with spastic cerebral palsy who met the inclusion criteria from October 2020 to March 2021 were included and randomly divided into observation group and control group, with 22 cases in each group. The control group received routine speech rehabilitation training, the observation group received real-time accent method on this basis. Each group received 10 times of training, 30 minutes each time, at most once a day, which should be completed within 3 weeks. The speech fluency was evaluated before and after the treatment. An adapted version of Singing standard sound by Liang Junbo was used as the test material. The differences of speech fluency in speech rate, pause, prolongation, and repetition were compared between the two groups under the task of imitation following language task, and the clinical efficacy was evaluated. Results Before treatment, there was no significant difference in speech rate, articulation rate, number of pauses, length of pauses, number of prolongations, length prolongations and repetitions between the two groups(all P>0.05), indicating that the two groups were comparable. After treatment, the seven indexes of speech fluency function of the two groups were statistically significant(all P<0.05), indicating that the speech fluency function of the observation group(were 2.08±0.15, 2.83±0.18, 18.41±1.62, 12.28±1.36, 17.59±2.04, 10.37±1.51, 3.00±1.23, respectively) was significantly better than that of the control group(were 1.60±0.06, 2.56±0.12, 35.23±3.13, 22.89±1.66, 37.05±3.27, 22.58±1.88, 5.18±1.74, respectively). After treatment, the seven indexes of speech fluency function in the observation group were significant better(all P<0.05) than those before treatment(were 1.55±0.15, 2.50±0.33, 36.55±3.57, 24.32±3.27, 35.50±4.15, 20.62±3.24, 5.82±2.72, respectively).
