摘  要:目的探讨急性后循环缺血对豚鼠蜗神经核能量代谢、SIRT1-PINK1介导的线粒体自噬的影响,以及电针的作用及机制。方法40只健康杂色豚鼠随机分为手术组、对照组、电针组和假手术组,每组10只,手术组和电针组豚鼠离断右侧颈总动脉、结扎右侧锁骨下动脉建立急性后循环缺血模型,电针组同时予以电针百会穴和内关穴,每次30分钟,每天1次,连续7天;假手术组暴露血管但不做离断和结扎;对照组自然喂养。采用Micro PET/CT检测双侧蜗神经核感兴趣区域摄取18F-FDG最大标准摄取值(maxmum standardized uptake values,SUVmax),以SUVmax左侧/右侧平均值分析各组葡萄糖代谢情况,ELISA方法检测ATP生成水平,实时荧光定量PCR法检测PINK1、SIRT1、LC3 mRNA的表达变化。结果与对照组比较,手术组SUVmax左/右平均比值升高、ATP水平降低;与手术组比较,电针组SUVmax左/右平均比值降低、ATP水平升高;电针组SIRT1 mRNA表达比手术组降低。各组PINK1 mRNA、LC3 mRNA的相对表达差异无统计学意义。结论电针百会穴和内关穴可能通过调节SIRT1的表达促进急性后循环缺血豚鼠蜗神经核能量代谢功能的重塑。Objective To explore the effects of acute posterior circulation ischemia(PCI)on energy metabolism and SIRT1-PINK1-mediated mitochondrial autophagy in cochlear nucleus of guinea pigs, and the effect and mechanism of electroacupuncture. Methods A total of 40 healthy variegated guinea pigs were randomly divided into four groups, including operation group(OG,n=10), control group(CG,n=10), electroacupuncture group(EOG,n=10) and sham operation group(SOG, n=10). The right common carotid artery was surgically dissected and the right subclavian artery was ligated in the guinea pigs of OG to build PCI model. In the EOG, electroacupuncturing at Baihui and Neiguan points was performed for 30 minutes every day, and lasted for 7 days after the artery operation as well as OG. Moreover, the carotid exposure was the same as OG, and was not dissected and ligated in SOG. Micro PET/CT was used to detect the level in glucose metabolism in the cochlear nucleus. The uptake value of 18 F-FDG in the left and right cochlear nucleus was detected by Micro PET/CT, and the average ratio of SUVmax left/right was analyzed in the cochlear nucleus. ELISA was used to detect the ATP level. The real-time quantitative PCR was used to detect the expression of PINK1, SIRT1 and LC3 mRNA. Results Compared with the CG, the average ratio of SUVmaxleft/rightincreased in the OG,and the ATP level decreased in the OG. Compared with the OG, the average ratio of SUVmaxleft/rightdecreased in the EOG,and the ATP level increased in the EOG. The expression of SIRT1 mRNA was lower in the EOG than that in the OG. There was no significant difference in the expression of PINK1 and LC3 mRNA among the 4 groups. Conclusion Electroacupuncturing at Baihui and Neiguan points may promote in the remodeling process of energy metabolism of cochlear nucleus in guinea pigs with acute PCI by regulating the expression of SIRT1.
