摘  要:目的本研究探讨激光散斑对比成像技术(LSCI)对585 nm脉冲染料激光治疗鲜红斑痣(PWS)前后的血流变化进行评价。方法选取20例PWS患者,给予585 nm脉冲染料激光治疗,治疗前、治疗后即刻及治疗后3个月分别接受3次LSCI检查,建立治疗前后PWS及正常皮肤血流散斑流速指数(SFI),治疗后3个月进行临床评估,并比较治疗前后PWSΔΔSFI与临床治疗疗效的相互联系。结果20例未治疗的PWS处SFI高于自身正常皮肤(P<0.01);治疗后皮损处即刻反应SFI下降,低于自身正常皮肤(P<0.001);治疗3个月后,皮损处SFI值仍高于正常皮肤,但较治疗前降低(P<0.05)。治疗前与治疗后PWS处ΔΔSFI与颜色消退正相关(r=0.72,P=0.013)。结论LSCI可客观量化监测PWS患者激光治疗前后微循环血流变化,是评估PWS治疗效果的一种有效、无创的检测方法。Objective The study utilizes the laser speckle contrast imaging(LSCI)to evaluate the changes of blood flow in port wine stain(PWS)before and after treatment with 585 nm pulsed dye laser.Methods Twenty patients with PWS were treated with 585 nm pulsed dye laser.LSCI was performed before,immediately and three months after the treatment respectively,the speckle flow index(SFI)of both PWS lesion and peripheral normal skin were established.The therapeutic efficacy was evaluated three months after the treatment.The correlation betweenΔΔSFI and therapeutic efficacy of PWS after laser treatment was compared.Results Compared with the normal skin of the same patient,the SFI of 20 untreated cases of PWS was significantly higher(P<0.01).Soon after treatment,SFI of the lesion was decreased compared with the normal skin(P<0.001).Three months after treatment,the SFI of the lesion was still higher than the normal skin,but differences were lower than before(P<0.05).There was a positive correlation betweenΔΔSFI of PWS with the effectiveness of the laser treatment(r=0.72,P=0.013).Conclusion LSCI can objectively and quantitatively monitor the changes of microcirculation blood flow in patients with PWS before and after laser treatment.It is an effective and non-invasive detection method to evaluate the therapeutic effect of PWS.
