多种肿瘤标志物联合检测在食管癌诊断中的价值

摘　　要：目的探讨鳞状细胞癌抗原(SCC)、癌胚抗原(CEA)、糖类抗原724(CA724)、糖类抗原199(CA199)及血管内皮生长因子(VEGF)5项肿瘤标志物联合检测对食管癌的诊断价值。方法选取2021年6月至2022年7月安阳市肿瘤医院住院患者102例,同期参加体检的健康成年人40例。比较两组研究对象血清中SCC、CEA、CA199、CA724及VEGF的表达水平,分析各项指标单独及联合检测的阳性率、灵敏度、特异度和约登指数,并制作受试者工作曲线评估各项指标单独及联合检测对食管癌的诊断价值。结果与对照组相比,食管癌组患者血清中SCC、CEA、CA199、CA724及VEGF的表达水平及阳性率升高,且SCC、CEA、CA199及VEGF显著升高(P<0.05)。联合检测阳性率显著高于单项检测阳性率(P<0.05)。结论SCC、CEA、CA199、CA724及VEGF5项指标联合检测对于食管癌的临床诊断有一定的预测效能,值得推广。Objective To explore the diagnostic value of combined detection of SCC,CEA,CA724,CA199 and VEGF in esophageal cancer Methods 102 patients(observation group)from June 2021 to July 2022 in our hospital,and 40 adults(control group)who participated in health examination during the same period were selected.The expression levels of SCC,CEA,CA199,CA724 and VEGF were compared between the two groups;the positive rates of single and combined tests were tested;the sensitivity,specificity and Jordan index of each index were analyzed;the ROC curve was made to evaluate the diagnostic value of each index in esophageal cancer.Results Compared with the control group,the expression level and positive rate of SCC,CEA,CA199,CA724 and VEGF in the serum of patients with esophageal cancer increased,and SCC,CEA,CA199 and VEGF increased significantly(P<0.05).The positive rate of combined detection of 5 indexes was significantly higher than that of single detection(P<0.05).Conclusion The combined detection of SCC,CEA,CA199,CA724 and VEGF has a certain predictive effect on the clinical diagnosis of esophageal cancer,which is worthy of promotion.
