伏诺拉生联合胶体果胶铋治疗老年2型糖尿病合并反流性食管炎的疗效

摘　　要：目的观察伏诺拉生联合胶体果胶铋治疗老年2型糖尿病(T2DM)合并反流性食管炎(RE)的疗效。方法选择2020年3月至2022年6月南阳市中心医院收治的老年T2DM合并反流性食管炎患者80例,用随机数字表法分为观察组和对照组各40例。比较患者治疗前、治疗30 d的炎症因子[肿瘤坏死因子-α(TNF-α)、单核细胞趋化蛋白1(MCP-1)、前列腺素e2(PGE2)]、胃肠激素[促胃液素(GAS)、促胰液素(SCT)、胃动素(MTL)]、食管24 h pH、治疗效果、食管黏膜溃疡愈合率与疾病复发率。结果治疗30 d,观察组和对照组TNF-α、MCP-1、PGE2、GAS、SCT、24 h食管pH<4总时长、pH<45 min反流次数、最长反流持续时间、Demeester总分均下降,观察组更低,差异有统计学意义(P<0.05);观察组和对照组MTL均升高,观察组更高,差异有统计学意义(P<0.05);观察组治疗总有效率、食管黏膜溃疡愈合率均比对照组高,复发率比对照组低,差异有统计学意义(P<0.05)。结论老年2型糖尿病合并反流性食管炎联合应用伏诺拉生与胶体果胶铋治疗,效果确切,可减轻机体炎症反应,调节胃肠激素和食管24 h pH,促进食管黏膜溃疡愈合,还可降低疾病复发风险。Objective To observe the effect of vonolaxan combined with colloidal bismuth in the treatment of senile T2 DM patients with reflux esophagitis(RE).Methods 80 elderly T2 DM patients with RE admitted to our hospital from March 2020 to June 2022,were selected and divided into the observation group and the control group randomly(40 cases in each).The inflammatory factors(TNF-α,MCP-1,PGE2),gastrointestinal hormones(GAS,SCT,MTL),24 h esophageal pH,healing rate of esophageal mucosal ulcer and recurrence rate in the two groups before treatment and 30 days after treatment,were compared.Results After 30 days of treatment,in the two groups TNF-α,MCP-1,PGE2,GAS,SCT,total duration of 24 h esophageal pH<4,the number of reflux times of pH<45 min,the longest duration of reflux,and the total score of Demeester were decreased significantly,which were lower in the observation group than those in the control group(P<0.05).The MTL of the two groups increased,and in the observation group the MTL was higher than that in the control group(P<0.05).The total effective rate and healing rate of the observation group was higher than those of the control group(P<0.05).The recurrence rate of the observation group was lower than that of the control group(P<0.05).Conclusion Vonolasan combined with bismuth colloidal pectin in the treatment of elderly T2 DM patients with RE can reduce the inflammatory response,regulate gastrointestinal hormones and 24 h pH of the esophagus,promote the healing of esophageal mucosal ulcer,and reduce the risk of disease recurrence.
