加味六味地黄颗粒联合常规治疗对老年高血压合并认知障碍患者的临床疗效

摘　　要：目的考察加味六味地黄颗粒联合常规治疗对老年高血压合并认知障碍患者的临床疗效。方法110例患者随机分为对照组(54例)和观察组(56例),在常规治疗(降压)基础上,对照组给予加味六味地黄颗粒模拟药,观察组给予加味六味地黄颗粒,疗程4个月。检测临床疗效、偶测血压达标率、认知功能指标(MoCA、MMSE评分)、日常生活能力指标(IADL、BADL评分)、动态血压(24 h SSD、24 h DSD、nSCV、nDCV)、肾阴不足证评分、血清蛋白标志物(Aβ1-42、tau总蛋白、P-tau)、血管炎性标志物(Lp-PLA2、PTX3、Hcy)、循环miR-126和miR-155、不良反应发生率变化。结果观察组总有效率、偶测血压达标率高于对照组(P<0.05)。治疗后,2组认知功能指标、日常生活能力指标、miR-126升高(P<0.05),动态血压、肾阴不足证评分、血清蛋白标志物、血管炎性标志物、miR-155降低(P<0.05),以观察组更明显(P<0.05)。2组未发现加味六味地黄颗粒相关不良反应。结论加味六味地黄颗粒联合常规治疗可安全有效地提高老年高血压合并认知障碍患者临床疗效,改善认知功能和日常生活能力,调节血清蛋白标志物和miR-126、miR-155水平,降低血管炎性因子表达。AIM To investigate the clinical effects of Supplemented Liuwei Dihuang Granules combined with conventional treatment on patients with hypertension in the elderly complicated with cognitive impairment.METHODS One hundred and ten patients were randomly assigned into control group(54 cases)for 4-month intervention of both model drug of Supplemented Liuwei Dihuang Granules and conventional treatment(anti-hypertension),and observation group(56 cases)for 4-month intervention of both Supplemented Liuwei Dihuang Granules and conventional treatment.The changes in clinical efficacy,compliance rate of occasionally measured blood pressure,cognitive function indices(MoCA,MMSE scores),daily living activity indices(IADL,BADL scores),ambulatory blood pressures(24 h SSD,24 h DSD,nSCV,nDCV),Kidney Yin Deficiency Syndrome score,serum protein markers(Aβ1-42,total Tau protein,P-tau),vascular inflammatory markers(Lp-PLA2,PTX3,Hcy),circulating miR-126 and miR-155 and incidence of adverse reactions were detected.RESULTS The observation group demonstrated higher total effective rate and compliance rate of occasionally measured blood pressure than the control group(P<0.05).After the treatment,the two groups displayed increased cognitive function indices,daily living activity indices,miR-126(P<0.05),and decreased ambulatory blood pressures,Kidney Yin Deficiency Syndrome score,serum protein markers,vascular inflammatory markers,miR-155(P<0.05),especially for the observation group(P<0.05).No adverse reactions related to Supplemented Liuwei Dihuang Granules were found.CONCLUSION For the patients with hypertension in the elderly complicated with cognitive impairment,Supplemented Liuwei Dihuang Granules combined with conventional treatment can safely and effectively enhance clinical efficacy,improve cognitive function and daily living ability,regulate the levels of serum protein markers,miR-126,miR-155,and reduce the expressions of vascular inflammatory factors.
