含佐太藏药复方制剂的研究

摘　　要：[目的]探讨6种国药准字含佐太藏药复方制剂的药理作用以及相关安全性问题。[方法]以佐太及各复方制剂的中英文为关键词,在中国知网、维普、万方、Web of Science、PubMed、Science Direct等数据库进行检索,查阅有关佐太及复方制剂的所有期刊文献,结合藏文版医学典籍的记载,对目前含佐太的藏药复方制剂的药理作用和安全性研究现状进行总结。[结果]佐太是八金八矿和许多天然药材经过特殊炮制而成的藏药,在藏医经典著作《四部医典》中,记载了100多种佐太的配方。佐太通常作为佐辅剂,与其他天然药物配伍,制成多种药效不同的复方制剂,以增效减毒。含佐太的藏药复方制剂具有改善中枢神经系统和消化系统功能、调节肠道菌群和非特异性免疫、辅助治疗癌症等作用。佐太中含有重金属,但是经过水银洗炼法炮制后,重金属毒性基本去除。因此在临床合理的使用下,含佐太藏药复方制剂是安全的,没有明显的急性毒性和长期毒性。使用含佐太的复方制剂会在肝肾组织中产生一定的汞蓄积,但停药后会逐步排出,故在肝肾内汞的蓄积是可逆的。[结论]佐太是珍宝类藏药中不可或缺的组分,对含佐太的藏药复方制剂应从原料辅料、炮制工艺、药理作用、毒理作用及临床评价等多个角度进行探索。[Objective]To discuss the pharmacological mechanism and the medication safety of six National Medical Products Administration(NMPA)Approval Tibet medicines containing Zuotai.[Methods]By searching China National Knowledge Infrastructure(CNKI),China Science and Technology Journal Database(VIP),Wanfang,Web of Science,PubMed and Science Direct database(up to now)with Zuotai and compound preparations in Chinese and English as key words,all the literature of Zuotai and compound preparations was retrieved.The mechanism and toxicity status of Tibet medicine containing Zuotai was summarized according to historical Tibetan medical books and contemporary academic papers.[Results]Zuotai is made of eight gold,eight minerals and many natural herbs,more than 100formulas of Zuotai are recorded in the Classical Book of Tibetan Medicine Rgyud bzhi.Zuotai is an adjuvant with other herb medicines to improve the curative effect and reduce toxicity.Compound preparations of Tibet medicine containing Zuotai could improve the function of central nervous system and digestive system,regularize the intestinal micro-ecology and nonspecific immunity,and auxiliary treatment of cancer.Zuotai contains heavy metals,but is processed by the technology of mercury washing method,the heavy metals in Zuotai losting its toxicity nearly,so these compound preparations containing Zuotai are safe and without obvious acute and longtime toxicity in reasonable clinical dosage.The compound preparations containing Zuotai will produce some mercury accumulation in the liver and kidney in medication period,but mercury will be gradually excreted after withdrawal,so the accumulation of mercury in the liver and kidney is reversible.[Conclusion]Zuotai is an indispensable component in precious Tibet medicine.The pharmaceutical value of Zuotai in Tibet medicine compound preparations should be explained in a variety of ways,such as the modern medical views of raw and auxiliary materials,processing technology,pharmacology,toxicology and clinical evaluation.
