绝经后骨质疏松并发骨关节炎患者采用骨化三醇联合阿仑膦酸钠治疗的疗效

摘　　要：目的探讨骨化三醇联合阿仑膦酸钠对绝经后骨质疏松并发骨关节炎患者骨代谢及骨强度的影响。方法选取2020年1月至2021年1月于安徽医科大学第一附属医院治疗的绝经后骨质疏松并发骨关节炎患者56例作为研究对象,按随机数字表法分为观察组与对照组,每组28例。对照组予以骨化三醇胶丸与维D钙咀嚼片口服,观察组患者在对照组用药基础上予以阿仑膦酸钠片口服治疗。治疗前及治疗12个月后检测两组患者腰椎骨密度(BMD)、股骨颈BMD及股骨颈屈曲应力比,酶联免疫吸附试验法检测骨代谢指标水平。并于治疗前、治疗6个月、12个月采用Lysholm膝关节评分评估两组患者膝关节功能。结果治疗前两组患者各项观察指标比较差异无统计学意义(P>0.05)。治疗12个月后观察组患者腰椎与股骨颈BMD较对照组明显升高,股骨颈屈曲应力比以及骨碱性磷酸酶、骨钙素水平显著降低,差异有统计学意义(P<0.05)。治疗6个月及12个月后观察组患者Lysholm膝关节评分均明显高于对照组(P<0.05)。结论对于绝经后骨质疏松并发骨关节炎患者,骨化三醇联合阿仑膦酸钠治疗有助于改善骨强度与骨骼几何结构的稳定性,调节骨代谢指标,有效改善关节功能。Objective To explore the effects of calcitriol combined with alendronate on bone metabolism and bone strength in postmenopausal osteoporosis patients with osteoarthritis.Methods A total of 56 postmenopausal patients with osteoporosis complicated with osteoarthritis who were treated in the First Affiliated Hospital of Anhui Medical University from January 2020 to January 2021 were selected.The patients were randomly divided into observation group and control group,with 28 patients in each group.The control group was orally treated with calcitriol capsule and calcium carbonate D3 tablets,while the observation group was orally treated with alendronate sodium tablets on the basis of medication in the control group.Lumbar bone density(BMD),femoral neck BMD and femoral neck flexion stress ratio were detected before treatment and 12 months after treatment,and bone metabolism indexes were detected by ELISA assay.Lysholm knee score was used to evaluate the knee function of 2 groups before treatment,6 and 12 months after treatment.Results There were no statistically significant differences in the two groups of patients before treatment(P>0.05).After 12 months of treatment,the BMD of lumbar spine and femoral neck in the observation group was significantly higher than that in the control group,while the flexural stress ratio of femoral neck and the levels of bone alkaline phosphatase and osteocalcin were significantly decreased(P<0.05).Lysholm knee score in the observation group was significantly higher than that in the control group after 6 and 12 months of treatment(P<0.01).Conclusion For postmenopausal patients with osteoporosis complicated with osteoarthritis,calcitriol combined with alendronate can improve the stability of bone strength and bone geometry,regulate bone metabolism and effectively improve joint function.
