摘 要:目的:探讨高危妊娠孕晚期监测脐血流收缩期峰值流速/舒张末期流速(S/D)比值在预测妊娠结局中的应用。方法:选取2020年10月-2021年10月嘉兴市妇幼保健院收治的90例孕晚期孕妇为研究对象,根据妊娠状态分为高危组与正常组,各45例。两组均完成超声检查,统计S/D比值及其他血流参数,比较孕妇S/D比值、阻力指数、搏动指数;以S/D值1.7分为界限,分为对照组与观察组,比较两组新生儿窒息率、宫内窘迫率、生长受限率。结果:高危组S/D比值、阻力、搏动指数均高于正常组,差异有统计学意义(P<0.05);观察组新生儿窒息率、宫内窘迫率、生长受限率均高于对照组,差异有统计学意义(P<0.05)。结论:彩超脐血流S/D比值可为预测高危妊娠产妇妊娠结局提供一定的理论依据。Objective: To investigate the application of monitoring the peak systolic flow rate/end-diastolic flow rate(S/D) ratio of umbilical blood flow in late pregnancy in high-risk pregnancies in predicting pregnancy outcomes. Methods: Ninety pregnant women admitted to Jiaxing Maternal and Child Health Hospital in late pregnancy from October 2020 to October 2021 were selected for the study, and were divided into a high-risk group and a normal group according to their pregnancy status, with 45cases each. Ultrasound examination was completed in both groups, and S/D ratio and other blood flow parameters were counted to compare maternal S/D ratio, resistance index, and pulsatility index;the S/D value of 1.7 was used as a cut-off to divide the control group and the observation group. The rates of neonatal asphyxia, intrauterine distress, and growth restriction were compared between the two groups. Results: The S/D ratio, resistance, and pulsatility index were higher in the high-risk group than in the normal group, and the differences were statistically significant(P<0.05). The neonatal asphyxia rate, intrauterine distress rate, and growth restriction rate in the observation group were higher than those in the control group, and the difference was statistically significant(P<0.05). Conclusion: The color ultrasound umbilical blood flow S/D ratio can provide some theoretical basis for predicting maternal pregnancy outcome in high-risk pregnancies.
