银屑病与2型糖尿病的多角度分析

摘　　要：银屑病是一种慢性炎症性皮肤病。近年来,研究者对银屑病发病机制的认识不断增加,人们开始关注银屑病与代谢之间的关系;而在各种代谢性疾病当中,2型糖尿病(T2DM)与银屑病关系较为密切,国内外诸多研究已经发现银屑病患者的2型糖尿病发生风险明显升高,且与银屑病的严重程度呈正相关。二者共同的病理生理变化即胰岛素抵抗,在银屑病患者体内参与胰岛素抵抗发生及发展的炎症因子异常表达,如TNF-α、IL-6、抵抗素和脂联素等。鉴于此,通过大量临床研究、基础研究发现,部分降糖药在控制血糖的同时也为银屑病的治疗带来获益,较为重要的有二甲双胍、噻唑烷二酮类、GLP-1RA,其机制是通过调节表皮细胞凋亡、抑制炎症细胞因子、改善脂代谢等方面对银屑病产生治疗作用。Psoriasis is a chronic inflammatory skin disease. In recent years,researchers have increased the understanding of the pathogenesis of psoriasis,and people began to pay attention to the relationship between psoriasis and metabolism. Among various metabolic diseases,type 2 diabetes mellitus(T2DM)is closely related to psoriasis. Many studies at home and abroad have found that the risk of developing type 2 diabetes in patients with psoriasis is significantly increased,and it is positively correlated with the severity of psoriasis. The common pathophysiological change is insulin resistance,and inflammatory factors involved in the occurrence and development of insulin resistance in patients with psoriasis are abnormally expressed,such as TNF-α,IL-6,resistin and adiponectin. In view of this,through a large number of clinical research,basic research,found that some medications to control blood sugar and at the same time bring benefit for the treatment of psoriasis. Metformin,Thiazolidinediones and GLP-1RA are more important,which have therapeutic effects on psoriasis through regulating epidermal cell apoptosis,inhibiting inflammatory cytokines and improving lipid metabolism.
