摘  要:目的了解2016-2020年南阳市土源性线虫感染情况,为制定南阳市“十四五”土源性线虫防控措施提供依据。方法2016-2020年,在南阳市11个县(市、区)的土源性线虫监测点,每个监测点采集≥3岁常住居民不少于1000人的粪便样本,采用改良加藤厚涂片法(Kato-Katz)检测其中的土源性线虫卵,并分类计数,计算感染率、感染度等指标。试管滤纸钩蚴培养法鉴定钩虫种类。结果2016-2020年南阳11个监测点开展调查,共调查11492人,土源性线虫总感染率为0.39%(45/11492),其中钩虫感染占86.67%(39/45),钩虫卵试管滤纸培养法鉴定均为美洲钩虫。社旗县土源性线虫感染率最高,为1.22%(13/1065),各监测点间感染率差异有统计学意义(χ^(2)=49.379,P<0.001)。土源性线虫感染率随着年龄的增长而升高(χ^(2)趋势=20.875,P<0.001);≥50岁人群钩虫感染率为0.73%(30/4091),高于<50岁人群的0.12%(9/7401),差异有统计学意义(χ^(2)=28.899,P<0.001)。不同文化程度人群感染率随着文化程度的增高而降低(χ^(2)趋势=5.278,P<0.05)。钩虫、蛔虫、鞭虫的感染率分别为0.34%(39/11492)、0.03%(3/11492)和0.03%(3/11492),且绝大部分为轻度感染。结论2016-2020年,南阳市监测点人群土源性线虫感染整体呈现较低水平,但高感染县(市、区)和传播风险依然存在,应加强土源性线虫病防治策略和措施研究,结合流行特征开展综合防控。Objective To understand the infection of soil-transmitted nematodes among residents in Nanyang from 2016 to 2020,and provide basis data for the formulation of the prevention and control plans of soil-transmitted nematodes during Fourteenth Five-Year Plan in Nanyang.Method A total of 11 surveillance sites were established in Nanyang from 2016 to 2020,and stool samples were collected from at least 1000 permanent residents over 3 years old at surveillance site.Each sample was examined microscopically using the modified Kato-Katz thick smear method for parasite eggs,which were counted for estimating infection rate and infection intensity.Hookworm eggs were examined by test-tube filter paper incubation for larva to identify the species.Results From 2016 to 2020,a total of 11492 people were investigated,and the average infection rate of soil-transmitted nematodes(hookworm,whipworm and Ascaris lumbricoides)was 0.39%(45/11492).Of the total infections,the proportion of hookworm infection was 86.67%(39/45).By Hookworm egg tube filter paper culture method,they were identified as Necator americanus.The infection rate of soil-transmitted nematodes in Sheqi county was the highest(1.22%,13/1065).There was significant difference in the infection rate among surveillance sites(χ^(2)=49.379,P<0.001).The infection rate increased with age(χ^(2)trend=20.875,P<0.001).Hookworm infection was 0.73%(30/4091)in the age group over 50 years old,which was higher than that of0.12%(9/7401)in the age group below 50 years old(χ^(2)=28.899,P<0.001).The infection rate decreased with the increase of educational level(χ^(2)trend=5.278,P<0.05).The infection rates of hookworm,Ascaris lumbricoides,and Trichuris trichiura were0.34%(39/11492),0.03%(3/11492)and 0.03%(3/11492),respectively,and the majority were of mild infection.Conclusion The overall infection rate of soil-transmitted nematodes in Nanyang from 2016 to 2020 was at a low level,but counties with higher infection rate and transmission risk remained,so the research on control strategies and
