基于数据挖掘探析颈源性失眠选穴规律

摘　　要：目的分析针灸治疗颈源性失眠的选穴规律。方法检索建库至2021年8月1日中国知网(CNKI)、万方学术期刊全文数据库(Wangfang)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)、PubMed、Wed of science、Embase收录的针灸治疗颈源性失眠的临床文献,采用Excel 2010归纳高频穴位、归经、部位,并用SPSS modeler 18.0、SPSS 20.0软件进行选穴的关联规则分析及聚类分析。结果共纳入文献61篇、针灸处方124条,涉及穴位84个;获高频穴位11个,依次为颈夹脊、神门、安眠、大椎、百会、风池、心俞、百劳、内关、照海、天柱,主要分布在颈部、头面部,以膀胱经及督脉为主,大致聚成3类,主要穴组为颈夹脊—风池—大椎—神门—安眠—百会—照海—心俞—内关。结论针灸治疗颈源性失眠取穴同取颈椎病腰穴和失眠要穴,以颈部穴位为主,侧重取颈夹脊穴,选用膀胱经和督脉,以治疗颈部疾患为主,辅以镇静、宁心、安神之穴位。Objective In order to analyze the acupoint selection rule of acupuncture and moxibustion in the treatment of cervical insomnia through data exploration.Methods The database was retrieved from the databases of CNKI,Wanfang,VIP,CBM,PubMed,Web of Science and Embase on August 1,2021,so as to collect clinical literature on acupuncture treatment of cervical insomnia.Excel2010 text database was used to summarize high-frequency acupoints,meridian return and location,and make a descriptive analysis.SPSS Modeler 18.0 and SPSS 20.0 statistical software were used for association rule analysis and cluster analysis.Analyze and summarize the law of acupoint selection.Results A total of 61 literatures were included,124 acupuncture prescriptions,involving 84 acupoints.Among them,there are 11 high-frequency acupoints,which are Jingjiaji,Shenmen,Anmian,Dazhui,Baihui,Fengchi,Xinshu,Bailao,Neiguan,Zhaohai and Tianzhu,which are mainly distributed in the neck,head and face.The main meridians are bladder meridians and governor meridians.The analysis of association rules shows that the core acupoint group is Jingjiaji-Fengchi-Dazhui-Shenmen-Anmian-Baihui-Zhaohai-Xinshu-Neiguan,High frequency acupoint clustering can be roughly divided into three categories.Conclusion The clinical acupuncture and moxibustion for cervical insomnia takes the same points as the waist point of cervical spondylosis and the key points of insomnia.It mainly takes the cervical acupoints,focuses on the Jingjiaji,selects the bladder meridian and Du pulse,which are mainly for the treatment of neck diseases,with sedation and tranquilizing points.
