推拿联合催乳汤与情志干预对剖宫产泌乳情况的影响

摘　　要：目的探讨推拿联合催乳汤与情志干预对剖宫产产妇泌乳情况的影响。方法选取2020年8月—2021年8月遂川县妇幼保健院收治的96例剖宫产产妇,随机分为对照组和观察组,每组48例。对照组采用催乳汤联合情志干预,观察组在此基础上加用推拿疗法,治疗周期均为7 d。比较2组产妇治疗后临床疗效、母乳喂养和新生儿睡眠情况,治疗前后泌乳量、补授乳量,产后72 h、7 d血清泌乳素、催产素水平。结果治疗后,观察组产妇的临床总有效率高于对照组;观察组治疗后妇泌乳量较对照组更多,且观察组补授乳量较对照组更少;观察组产后72 h、7 d的血清泌乳素和催产素水平均高于对照组;观察组产妇治疗后的母乳喂养率、新生儿睡眠正常率均优于较对照组(均P<0.05)。结论中医推拿联合催乳汤与情志干预可有效提高剖宫产产妇的泌乳量和母乳喂养率,还可有效改善新生儿睡眠质量,促进产后乳汁分泌,且疗效显著。Objective To explore the effect of traditional Chinese medicine massage combined with Cuiru decoction and emotional intervention on lactation of cesarean section women.Methods A total of 96 women with cesarean section admitted to Suichuan Maternal and Child Health Hospital from August 2020 to August 2021 were selected and randomly divided into control group and observation group,with 48 cases in each group.They were grouped according to the random number table.The control group was treated with Cuiru decoction combined with emotional intervention,and the observation group was treated with massage therapy on this basis.The treatment period was 7 days.The clinical effect,breastfeeding and neonatal sleep,lactation volume and supplemental lactation volume before and after treatment,serum prolactin and oxytocin levels at 72 h and 7 d after delivery were compared between the two groups.Results After treatment,the total effective rate of observation group was higher than that of control group.After treatment,the lactation volume of the observation group was more than that of the control group,and the supplemental lactation volume of the observation group was less than that of the control group.The serum prolactin and oxytocin levels in the observation group were higher than those in the control group at 72 h and 7 d after delivery.The breastfeeding rate and normal sleep rate of neonates in the observation group after treatment were better than those in the control group(all P<0.05).Conclusion Traditional Chinese medicine massage combined with Cuiru decoction and emotional intervention can effectively improve the lactation volume and breastfeeding rate of cesarean section women,but also can effectively improve the sleep quality of newborns,promote postpartum milk secretion,and the effect is significant.
