柴胡郁金排石汤与硬膜外腔阻滞麻醉治疗胆总管结石临床观察

摘　　要：目的探讨柴胡郁金排石汤与硬膜外腔阻滞麻醉治疗胆总管结石的效果。方法确山县人民医院2019年4月—2021年4月期间收治的胆总管结石患者中选出68例作为本次研究的研究对象,并采用随机数字表法将该68例胆总管结石患者分为对照组(34例)与观察组(34例)。对照组采用硬膜外腔阻滞麻醉穿刺术治疗,观察组在对照组基础上使用柴胡郁金排石汤,治疗14 d,2组均观察至出院。比较2组治疗后临床疗效、胆汁引流量、住院时间,治疗前后血清生化指标水平及治疗期间不良反应。结果治疗期间,观察组不良反应总发生率低于对照组;观察组胆汁引流量多于对照组,住院时间短于对照组;与治疗前2组血清ALT、GGT、DB、TB水平比较,治疗后均为降低状态,且观察组各项水平较对照组均处于更低水平;治疗后观察组总有效率高于对照组(均P<0.05)。结论柴胡郁金排石汤与硬膜外腔阻滞麻醉治疗胆总管结石能够减轻患者机体胆汁淤积程度,改善肝胆功能,降低不良反应发生率,提高临床疗效。Objective To explore the curative effect of Chaihu Yujin Paishi decoction and epidural block anesthesia in the treatment of choledocholithiasis.Methods A total of 68 patients with common bile duct stones who were admitted to Queshan County People’s Hospital from April 2019 to April 2021 were selected as the research subjects of this study,and the 68 patients with common bile duct stones were divided into control group(34 cases)and observation group(34 cases)using a random number table method.The control group was treated with epidural anesthesia and puncture,and the observation group was treated with Chaihu Yujin Paishi decoction on the basis of the control group for a total of 14 days.Both groups were observed until discharged.The clinical efficacy after treatment,bile drainage,hospitalization time,serum biochemical index levels before and after treatment,and adverse reactions during treatment were compared.Results During treatment,the total incidence of adverse reactions in the observation group,which was lower than the control group.The observation group had more bile drainage than the control group,and the hospital stay was shorter than that of the control group.Compared with the serum ALT,GGT,DB,and TB levels of the two groups before treatment,they were all reduced after treatment,and all levels of the observation group were lower than those of the control group.The total effective rate of the observation group after treatment was higher than that of the control group(all P<0.05).Conclusion Chaihu Yujin Paishi decoction and epidural block anesthesia in the treatment of choledocholithiasis can reduce the degree of cholestasis in the patient’s body,improve liver and gallbladder function,reduce the incidence of adverse reactions,and improve clinical effect.
