颊针联合穴位注射治疗鼻咽癌放化疗后听力损伤的疗效观察

摘　　要：【目的】观察颊针联合穴位注射治疗鼻咽癌放化疗后听力损伤患者的临床疗效。【方法】将44例鼻咽癌放化疗后听力损伤患者随机分为治疗组和对照组,每组各22例。对照组患者给予口服甲钴胺片治疗,治疗组患者在对照组的基础上给予颊针(针刺头穴、颈穴、上颈穴)联合穴位注射(在听宫穴和翳风穴处注射甲钴胺注射液)治疗,疗程为4周。观察2组患者治疗前及治疗4周后的纯音听力测试(PTA)值和耳鸣残疾评估量表(THI)评分的变化情况,以评价2组患者的听力改善情况和耳鸣改善情况。【结果】(1)听力改善情况:治疗4周后,2组患者的左右听力PTA值均较治疗前有所改善(P<0.05);但治疗后组间比较,差异均无统计学意义(P>0.05),表明观察组患者的听力改善程度与对照组相当。(2)耳鸣改善情况:治疗4周后,2组患者的耳鸣THI评分均较治疗前改善(P<0.05),且观察组患者对耳鸣THI评分的改善作用明显优于对照组,差异有统计学意义(P<0.01)。【结论】颊针联合穴位注射治疗鼻咽癌放化疗后造成的耳鸣患者疗效显著,同时可以有效减轻和减缓鼻咽癌放化疗后所造成的听力损害。Objective To observe the clinical efficacy of buccal needling combined with acupoint injection in treating hearing impairment after chemoradiotherapy for nasopharyngeal carcinoma(NPC).Methods Forty-four patients with hearing impairment after chemoradiotherapy for NPC were randomly divided into treatment group and control group,with 22 patients in each group.The patients in the control group were given oral use of Mecobalamin Tablets,while the patients in the treatment group were treated with buccal needling(acupuncture of the points of the head,neck and upper neck)combined with acupoint injection[Mecobalamin injection at the Tinggong point(SI19)and Yifeng point(SJ17)]on the basis of treatment for the control group.The treatments for both groups lasted for 4 weeks.The changes in pure tone audiometry(PTA)value and Tinnitus Handicap Inventory(THI)scores of the two groups were observed before treatment and after 4 weeks of treatment,so as to evaluate the improvement of hearing and tinnitus in both groups.Results(1)After 4 weeks of treatment,the PTA values of the left and right ear of patients in both groups were improved compared with those before treatment(P<0.05).However,there was no statistically significant difference between the groups after treatment(P>0.05),indicating that the hearing improvement in the observation group was similar to that in the control group.(2)After 4 weeks of treatment,the tinnitus THI scores of patients in both groups were improved compared with those before treatment(P<0.05).The intergroup comparison showed that the improvement of tinnitus THI scores in the observation group was significantly superior to that in the control group,and the difference was statistically significant(P<0.01).Conclusion Buccal needling combined with acupoint injection exerts significant effect in treating patients with tinnitus caused by chemoradiotherapy for NPC,which is also effective in reducing and relieving the hearing impairment caused by chemoradiotherapy for NPC.
