基于数据挖掘探讨丘和明治疗慢性再生障碍性贫血的用药经验

摘　　要：采用频数分析、关联规则分析、聚类分析等数据挖掘方法对丘和明教授门诊治疗慢性再生障碍性贫血患者的有效病例的处方资料进行分析,以探讨其治疗慢性再生障碍性贫血的组方及用药规律。共录入126首处方,涉及中药79味,其中使用频次≥10次的中药有24味,居前5位的药物为山药、仙鹤草、山茱萸、熟地黄、龟甲;常用药物组合有“山药-山茱萸”“山药-熟地黄”等;核心处方药物包括熟地黄、山茱萸、仙鹤草、山药、鹿角胶、菟丝子、枸杞子、龟甲、党参、神曲、地稔、炙甘草12味药。分析结果提示,丘和明教授治疗慢性再生障碍性贫血的用药以平和、甘苦之药为主,处方精简,药性平和;治疗原则首重滋肾填精,以左归丸为基础方,辅以健脾益气、凉血止血之品;擅长运用仙鹤草、地稔等岭南特色药材;同时注重标本兼治,必要时酌加祛风除湿等祛邪药物。Data mining methods such as frequency analysis,association rule analysis and cluster analysis were used to analyze the prescription information of effective cases of outpatients with chronic aplastic anemia treated by Professor QIU He-Ming.And the rules of prescription and medication for chronic aplastic anemia treated by Professor QIU He-Ming were explored.A total of 126 prescriptions were included,involving 79 Chinese medicines.Among the included herbs,24 Chinese medicines were used more than 10 times,and the top 5 medicines were Rhizoma Dioscoreae,Herba Agrimoniae,Fructus Corni,Radix Rehmanniae Praeparata and Carapax et Plastrum Testudinis.The commonly-used herbal combinations were“Rhizoma Dioscoreae-Fructus Corni”and“Rhizoma Dioscoreae-Radix Rehmanniae Praeparata”.The core prescription included 12 herbs,and they were Radix Rehmanniae Praeparata,Fructus Corni,Herba Agrimoniae,Rhizoma Dioscoreae,Colla Cornus Cervi,Semen Cuscutae,Fructus Lycii,Carapax et Plastrum Testudinis,Radix Codonopsis,Massa Medicata Fermentata,Herba Melastomatis Dodecandri and Radix Glycyrrhizae Praeparata.The results of the analysis suggest that the herbs used for the treatment of chronic aplastic anemia by Professor QIU He-Ming are mild in nature,and sweet and bitter in taste,and the prescriptions are concise in the composition and mild in nature.The therapeutic principle for chronic aplastic anemia is to nourish kidney and replenish essence,with Zuogui Pills as the basic formula,supplemented by the herbs of strengthening spleen and replenishing qi,cooling blood to arrest bleeding.Professor QIU He-Ming is good at using the original Lingnan herbs of Herba Agrimoniae and Herba Melastomatis Dodecandri,and stresses the simultaneous treatment of the primary and secondary symptoms,and the application of medicines for dispelling wind and eliminating dampness when necessary.
