子痫前期孕妇血清胎盘生长因子可溶性血管内皮生长因子受体-1与妊娠结局的关系

摘　　要：目的探讨子痫前期孕妇血清胎盘生长因子(PLGF)、可溶性血管内皮生长因子受体-1(sFLT-1)及sFLT-1/PLGF水平与妊娠结局的关系。方法分析2019年12月至2022年1月福建省立金山医院妇产科产检并完成分娩的106例子痫前期孕妇的临床资料,根据妊娠结局分为不良妊娠结局组和良好妊娠结局组;检测106例子痫前期孕妇血清中PLGF、sFLT-1,并计算sFLT-1/PLGF比值,比较不良妊娠结局组与良好妊娠结局组血清PLGF、sFLT-1及sFLT-1/PLGF水平上的差异;并以受试者工作特征(ROC)曲线评价其诊断效能。结果不良妊娠结局组sFLT-1、sFLT-1/PLGF高于良好妊娠结局组(P<0.05),不良妊娠结局组PLGF低于良好妊娠结局组(P<0.05)。PLGF最佳截断值为114.1 pg/ml,敏感度为0.703,特异度为0.690;sFLT-1最佳截断点为8581 pg/ml,敏感度为0.476,特异度为0.906;sFLT-1/PLGF约最佳截断值为81.468,敏感度为0.571,特异度为0.812。结论子痫前期孕妇血清PLGF、sFLT-1及sFLT-1/PLGF水平与妊娠结局有关,对子痫前期孕妇妊娠结局均具有一定的预测效能。Objective To investigate the relationship between serum levels of placental growth fa-ctor(PLGF),soluble fms-like tyrosine kinase-1(sFLT)-1 and sFLT-1/PLGF in pregnant women with preecla-mpsia and pregnancy outcomes.Methods The clinical data of 106 pregnant women with preeclampsia who completed the obstetrics and gynecology examination in our hospital from December 2019 to January 2022 were analyzed.Serum levels of PLGF and sFLT-1 in pregnant women were calculated,and the ratio of sFLT-1/PLGF was calculated,and the differences in serum PLGF,sFLT-1 and sFLT-1/PLGF levels between the poor pregnancy outcome group and the good pregnancy outcome group were compared;Spearman correlation analysis was used.The relationship of PLGF,sFLT-1 and sFLT-1/PLGF with pregnancy outcomes were analyzed,and their diagnostic performance was evaluated by receiver operating characteristic(ROC)curve.Results The sFLT-1 and sFLT-1/PLGF in the adverse pregnancy outcome group were significantly higher than those in the good pregnancy outcome group(P<0.05),and the PLGF in the adverse pregnancy outcome group was significantly lower than that in the good pregnancy outcome group(P<0.05).The best cut-off point of PLGF was 114.1 pg/ml,the sensitivity was 0.703,and the specificity was 0.690;the best Cut-off point of sFLT-1 was 8581 pg/ml,the sensitivity was 0.476,and the specificity was 0.906;The optimal Cut-off point was 81.468,the sensitivity was 0.571,and the specificity was 0.812.Conclusion The serum levels of PLGF,sFLT-1 and sFLT-1/PLGF in pregnant women with preeclampsia are associated with pregnancy outcomes,and they all have certain pre-dictive power for pregnancy outcomes in pregnant women with preeclampsia.
