摘　　要：目的检测100例淋巴瘤患者中潜伏膜蛋白1(LMP1)及EBV编码的小RNA(EBER)的表达情况;观察LMP1与EBER的相关性及二者与淋巴瘤类型的相关性,为淋巴瘤的诊断提供依据。方法采用免疫组化SP法和原位杂交法分别检测100例淋巴瘤患者中LMP1和EBER的表达情况,比较两种方法的敏感性。结果(1)仅经典型霍奇金淋巴瘤、弥漫性大B细胞淋巴瘤表达LMP1和EBER及NK/T细胞淋巴瘤表达EBER,其余类型淋巴瘤中非肿瘤细胞可散在表达EBER。并且经典型霍奇金淋巴瘤LMP1表达率明显高于弥漫性大B细胞淋巴瘤(P<0.05);从EBER阳性表达率看,NK/T细胞淋巴瘤最高,霍奇金淋巴瘤次高,二者均明显高于弥漫性大B细胞淋巴瘤(均P<0.05)。在所有患者中EBER的表达率明显高于LMP1(P<0.05)。(2)从LMP1及EBER表达模式看,LMP1阳性的肿瘤细胞通常散在分布,而EBER阳性的肿瘤细胞通常呈簇状或片状分布、甚至弥漫分布。并且不同类型淋巴瘤EBER的表达模式也不同。结论EBV的表达与NK/T细胞淋巴瘤、经典型霍奇金淋巴瘤密切相关,并且可以作为辅助诊断此两种淋巴瘤的依据。并且弥漫性大B细胞淋巴瘤可伴有一定比例的EBV感染。EBER原位杂交的检测方法较为敏感并且EBER的表达模式有助于淋巴瘤类型的判定。Objective To detect the expression of LMP1and EBER in 100cases of lymphoma patients for screening EBV-positive lymphoma cases;To observe the correlation between LMP1and EBER and that between the expression of them and the type of lymphoma for providing the basis for the diagnosis of lymphoma.Methods Immunohistochemical SP method and in situ hybridization were used to detect the expression of LMP1and EBER in 100cases of lymphoma patients,respectively;the sensitivity of the two methods was compared.Results Only classical Hodgkin’s lymphoma,diffuse large B-cell lymphoma expressed LMP1and EBER and NK/T cell lymphoma expressed EBER.Non-tumor cells in other types of lymphoma can be scattered in the expression of EBER.And the expression rate of LMP1in classical Hodgkin’s lymphoma was significantly higher than that in diffuse large B-cell lymphoma(P<0.05).From the positive expression rate of EBER,NK/T cell lymphoma was the highest,and Hodgkin’s lymphoma was the second highest,both of which were significantly higher than diffuse large B-cell lymphoma(both P<0.05).The expression rate of EBER was significantly higher in all patients than in LMP1(P<0.05).(2)From the LMP1and EBER expression patterns,LMP1-positive tumor cells are usually scattered,and EBER-positive tumor cells are usually clustered or flaky,even diffusely distributed.And the expression patterns of EBER in different types of lymphoma are also different.Conclusion The expression of EBV is closely related to NK/T cell lymphoma and classical Hodgkin’s lymphoma,and can be used as a basis for the diagnosis of these two lymphomas.And diffuse large B-cell lymphoma can be associated with a certain proportion of EBV infection.The detection method of in situ hybridization is sensitive and the expression pattern of EBER contributes to the determination of lymphoma type.
