血清可溶性CD40L、可溶性细胞间黏附因子、 C反应蛋白以及血清胆红素水平与经皮冠状动脉介入术后血管再狭窄的相关性分析

摘　　要：目的 分析可溶性CD40L(sCD40L)、可溶性细胞间黏附因子(sICAM-1)、C反应蛋白(hs-CRP)及总胆红素水平与冠心病术后血管再狭窄的相关性。方法 选取南阳市中心医院经皮冠状动脉介入术治疗的125例患者,在手术后1年进行回访,根据造影结果分为术后再狭窄组与术后非狭窄组,比较两组血清指标水平并分析sCD40L、sICAM-1、hs-CRP、血清总胆红素与血管再狭窄的相关性。结果 术后再狭窄组患者血清sCD40L、sICAM-1、hs-CRP水平升高,血清总胆红素水平降低,差异有显著性(P<0.05);经过Logistic回归分析术后再狭窄组患者血清sCD40L、sICAM-1、hs-CRP、血清总胆红素水平是冠心病经皮冠状动脉介入治疗术后发生血管再狭窄的独立危险因素;通过Spearman分析,血管再狭窄与血清sCD40L、sICAM-1、hs-CRP水平呈正相关(r=0.774、0.729、0.505),血管再狭窄与总胆红素水平变化呈负相关(r=-0.683)。ROC曲线发现血清sCD40L、sICAM-1、hs-CRP、血清总胆红素均有预测价值(0.998、0.969、0.825、0.939)。结论 血清sCD40L、sICAM-1、hs-CRP、总胆红素是术后发生再狭窄的危险因素,具有重要预测价值。Objective To analyze the correlation between the levels of soluble CD40 L(sCD40 L),soluble intercellular adhesion factor-1(sICAM-1),C-reactive protein(hs-CRP) and total bilirubin and restenosis after coronary heart disease surgery.Methods 125 patients with PCI in our hospital were selected and followed up one year after operation.According to the results of angiography, they were divided into restenosis group and non restenosis group.The serum levels of sCD40 L,sICAM-1,hs-CRP and serum total bilirubin were compared between the two groups.Results The serum levels of sCD40 L,sICAM-1 and hs-CRP were increased and the serum total bilirubin was decreased in the restenosis group(P<0.05);Logistic regression analysis showed that the serum levels of sCD40 L,sICAM-1,hs-CRP and total bilirubin were independent risk factors for restenosis after PCI;Spearman analysis showed that vascular restenosis was positively correlated with serum levels of sCD40 L,sICAM-1 and hs-CRP(r=0.774,0.729 and 0.505),while vascular restenosis was negatively correlated with total bilirubin(r=-0.683).ROC curve showed that serum sCD40 L,sICAM-1,hs-CRP and total bilirubin had predictive value(0.998,0.969,0.825,0.939).Conclusion serum sCD40 L,sICAM-1,hs-CRP and total bilirubin are risk factors of restenosis after operation, which has important predictive value.
