摘  要:目的探讨宫颈液基细胞学、DNA倍体检测和HPV分型检测联合检查对提高宫颈癌筛查检出效能的影响。方法对2016年11月—2019年11月100例于东莞市人民医院和东莞康华医院接受宫颈癌筛查女性宫颈液基细胞学、DNA倍体检测和HPV分型检测,并对阳性结果患者进行病理组织学检查比对,分析宫颈液基细胞学、DNA倍体检测和HPV分型检测单独和联合应用对宫颈癌、癌前病变的筛查检出效能。结果病理组织学检查结果显示,100例宫颈癌筛查女性中癌变患者3.00%(3.00/100),癌前病变女性35.00%(35/100)。宫颈液基细胞学的宫颈癌诊断的敏感度、特异度和准确性分别为100.00%、82.47%和83.00%,癌前病变诊断的敏感度、特异度和准确性分别为45.71%、93.85%和77.00%。DNA倍体检测的宫颈癌诊断的敏感度、特异度和准确性分别为100.00%、67.01%和68.00%,癌前病变诊断的敏感度、特异度和准确性分别为91.43%、95.38%和94.00%。HPV分型检测的宫颈癌诊断的敏感度、特异度和准确性分别为100.00%、83.51%和84.00%,癌前病变诊断的敏感度、特异度和准确性分别为42.86%、93.85%和76.00%。宫颈液基细胞学、DNA倍体检测和HPV分型检测联合应用宫颈癌诊断的敏感度、特异度和准确性分别为100.00%、93.81%和94.00%,癌前病变诊断的敏感度、特异度和准确性分别为100.00%、96.92%和98.00%。宫颈液基细胞学、DNA倍体检测和HPV分型检测联合应用宫颈癌诊断的敏感度、特异度和准确性高于其单独应用(P<0.05)。结论宫颈液基细胞学、DNA倍体检测和HPV分型检测联合检测可在一定程度上提高宫颈癌筛查检出效能。Objective To investigate the cervical cancer screening improvment effect of cervical fluid based cytology,DNA ploidy combined with HPV typing test.Methods From November 2016 to November 2019,100 women who voluntarily underwent cervical cancer screening in Dongguan People’s Hospital and Dongguan Kanghua hospital were tested for cervical liquid-based cytology,DNA ploidy and HPV typing,and the patients with positive results were compared by histopathological examination.The cervical fluid based cytology,DNA ploidy test and HPV typing test were analyzed for cervical cancer and precancerous lesions Screening efficiency.Results The results of histopathological examination showed that among 100 women screened for cervical cancer,3.00%(3.00/100)were cancerous,35.00%(35/100)were precancerous.The sensitivity,specificity and accuracy of cervical cancer diagnosis of cervical liquid-based cytology were 100.00%,82.47% and 83.00%,respectively,and the sensitivity,specificity and accuracy of precancerous lesions were 45.71%,93.85% and 77.00% respectively.The sensitivity,specificity and accuracy of DNA ploidy detection for cervical cancer diagnosis were 100.00%,67.01% and 68.00%,respectively,and the sensitivity,specificity and accuracy for the diagnosis of precancerous lesions were 91.43%,95.38% and 94.00%,respectively.The sensitivity,specificity and accuracy of HPV typing detection for cervical cancer diagnosis were 100.00%,83.51% and 84.00%,respectively,and the sensitivity,specificity and accuracy for the diagnosis of precancerous lesions were 42.86%,93.85% and 76.00%,respectively.The combined application of cervical liquid-based cytology,DNA ploidy detection and HPV typing detection had the sensitivity,specificity and accuracy of cervical cancer diagnosis of 100.00%,93.81% and 94.00%,respectively,and the sensitivity and specificity accuracy of precancerous lesions diagnosis and accuracy were 100.00%,96.92% and 98.00%,respectively.The sensitivity,specificity and accuracy of cervical fluid based cytology,DNA ploidy combined wit
