超声引导下局部针刺联合复方倍他米松注射液治疗足底筋膜炎的临床价值

摘　　要：目的:探讨超声引导下局部针刺联合复方倍他米松注射液治疗足底筋膜炎的临床价值。方法:将57例足底筋膜炎患者分为两组,A组(29例)接受超声引导下局部针刺联合复方倍他米松注射液治疗,B组(28例)接受物理治疗及拉伸训练。在治疗前、治疗2周及4周,均通过视觉模拟评分法(Visual analogue scale,VAS)记录疼痛分值,并利用超声测量足底筋膜的厚度,比较两组受试者治疗前及治疗后VAS疼痛评分及足底筋膜厚度的变化情况。结果:治疗前、治疗2周及4周,A组VAS疼痛评分平均值分别为5.28±1.16、1.34±0.77、0.38±0.49,B组分别为5.32±1.12、3.82±1.19、3.00±1.31;A组足底筋膜厚度平均值分别为(5.48±0.73)mm、(3.94±0.22)mm、(3.72±0.19)mm,B组分别为(5.35±0.80)mm、(4.89±0.75)mm、(4.53±0.69)mm。治疗前,两组数值组间比较无明显差异(P>0.05);治疗2周及4周,A组的VAS疼痛评分、足底筋膜厚度平均值均低于B组,组间比较差异明显(P<0.001);A组疼痛评分及足底筋膜厚度较B组下降趋势更显著,A组的治疗效果优于B组。结论:超声引导下局部针刺联合复方倍他米松注射液治疗足底筋膜炎优于物理治疗及拉伸训练,具有良好的临床推广价值。Objective:To investigate the clinical value of ultrasound guided local acupuncture combined with compound betamethasone injection in the treatment of plantar fasciitis.Methods:Fifty-seven patients with plantar fasciitis were divided into two groups:group A(n=29)received local acupuncture combined with compound betamethasone injection,and group B(n=28)received general physical therapy.Before treatment,at 2 and 4 weeks,pain scores were recorded by visual analogue scale(VAS),and plantar fascia thickness was measured by ultrasound.The changes of VAS score and plantar fascia thickness were compared between the two groups before and after treatment.Results:The VAS scores of group A were 5.28±1.16,1.34±0.77,0.38±0.49 before treatment,and those of group B were 5.32±1.12,3.82±1.19,3.00±1.31 after treatment for 2 and 4 weeks.The average plantar fascia thickness were(5.48±0.73)mm,(3.94±0.22)mm,(3.72±0.19)mm in group A and(5.35±0.80)mm,(4.89±0.75)mm,(4.53±0.69)mm in group B.Before treatment,there was no significant difference between the two groups(P>0.05).After 2 and 4 weeks of treatment,VAS score and average plantar fascia thickness in group A were significantly lower than those in group B(P<0.001).Different groups had different effects on VAS pain score and plantar fascia thickness(P<0.001).Compared with group B,VAS score and plantar fascia thickness in group A decreased significantly,and the treatment effect of group A was better than group B.Conclusion:Ultrasound-guided local acupuncture combined with compound betamethasone injection is superior to physical therapy and stretching training,which has good clinical promotion value.
