70岁以上老龄语后聋患者国产人工耳蜗植入术后效果回顾性研究

摘　　要：目的评估重度或极重度感音神经性聋老龄患者国产人工耳蜗植入术后听觉康复效果,分析相关影响因素。方法回顾性分析27位70岁及以上老龄语后聋人工耳蜗植入患者资料,根据每天佩戴人工耳蜗时间分为两组。对每组手术前后CAP值、性别、植入耳侧别、耳聋时间、年龄、随访时间对围手术期及术后康复行为进行统计学分析。结果纳入被试平均年龄75.4岁,均行单耳植入,其中13例左耳,14例右耳。1例在术后9个月因反复头晕伴焦虑取出人工耳蜗。其余26例无严重并发症发生,随访9~24月,每组术后平均CAP显著高于术前(P<0.05)。结论70岁及以上老龄语后聋患者可安全接受国产人工耳蜗植入术,根据个体差异进行个性化手术,患者的认知水平及家庭支持越高,佩戴时间越长越规律,听觉康复效果越满意。Objective To evaluate the effect of hearing rehabilitation after cochlear implantation in aged patients with severe or profound sensorineural hearing loss,and to analyze the influencing factors.Methods The data of 27 deaf patients aged 70 years and above with cochlear implantation were analyzed retrospectively,and the rehabilitation behaviors were evaluated,and auditory perception was assessed using the categories of auditory performance(CAP).Results 22 patients(70-79 years old),3 patients(80-89 years old),and 2 patients(91 years old),with an average age of 75.4 years,underwent single ear implantation,including 13 left ear and 14 right ear.One patient underwent cochlear implantation 9 months after operation due to recurrent dizziness and anxiety.The remaining 26 patients were followed up for 9-24 months and divided into two groups according to the time of daily use of cochlear implantation.13 patients(Group 1)wore the CI more than 8 hours/day,the mean of CAP before operation was 1.38±0.87.The mean after operation was 5.23±1.09,the difference was statistically significant(P<0.05).The other 13 patients(Group 2)wore CI irregularly,the mean of CAP before operation was 1.54±1.33,and the mean after operation was 2.92±1.04.The difference was statistically significant(P<0.05).There was no significant difference between the two groups in gender,ear side,time of deafness,age and follow-up time(P>0.05),no significant difference in average CAP before operation(P>0.05),and no significant difference in average CAP after operation(P<0.05).Conclusion The deaf patients aged 70 years and above can safely accept the home-made cochlear implantation,according to individual difference,the higher the cognitive level and family support,the longer the wearing time,the more regular,the more satisfied the auditory rehabilitation effect.
