摘　　要：目的 探讨新生儿经外周静脉穿刺中心静脉置管(PICC)后48 h首次换药对预后并发症、穿刺点愈合的影响。方法 选取2021年6月至2022年2月广东省茂名市妇幼保健院收治的110例PICC置管新生儿作为研究对象,按照随机数字表法分为研究组(55例)和对照组(55例)。对照组首次换药时间为PICC置管后24 h,研究组首次换药时间为置管后48 h。比较两组患儿的穿刺并发症发生情况、局部渗血情况及穿刺点愈合情况。结果 两组患儿的穿刺并发症总发生率比较,差异无统计学意义(P>0.05);研究组患儿的局部渗血情况优于对照组,穿刺部位完全愈合时间短于对照组,差异有统计学意义(P<0.05)。结论 在新生儿PICC置管后48h换药的安全性更高,对降低患儿穿刺并发症发生风险、促进穿刺点愈合均有积极意义,值得推广。Objective To investigate the effect of the first dressing change 48 hours after peripheral venipuncture central venous catheter(PICC) on the complications of prognosis and puncture site healing in neonates. Methods From February2021 to December 2021, 110 neonates with PICC intubation who were admitted to the Maternal and Child Health Hospital of Maoming City, Guangdong Province were selected as the research objects, and divided into the study group(55 cases) and the control group(55 cases) according to the random number table method. The first dressing change time in the control group was 24 h after PICC catheter placement, and the first dressing change time in the study group was 48 h after catheter placement. The incidence of puncture complications, local bleeding and puncture site healing were compared between the two groups. Results There was no significant difference in the total incidence of puncture complications between the two groups(P>0.05). The local bleeding in the study group was better than that in the control group, and the complete healing time of puncture site was shorter than that of control group, the differences were statistically significant(P<0.05). Conclusion Dressing change 48 hours after neonatal PICC has higher safety, and has positive significance in reducing the risk of puncture complications and promoting the healing of puncture sites, which is worthy of promotion.
