摘　　要：目的分析我院住院患者丙戊酸(Valproic Acid,VPA)血药浓度测定结果及其影响因素,为临床的合理用药提供参考。方法收集我院2019~2021年住院期间进行丙戊酸血药浓度监测的患者共203例,采用单因素分析和多重线性回归方法,探讨年龄、性别、体重、肝功能、血肌酐、白蛋白、剂型及合并用药等因素与VPA血药浓度/剂量比(Concentration/Dose,以“C/D”表示)的相关性。结果单因素分析结果提示年龄、性别、体重、VPA剂型、是否合并使用头孢哌酮舒巴坦、是否合并使用脱水药与VPA的C/D均有相关性,而多因素分析仅保留了性别、VPA剂型两个为独立因素。结论VPA的血药浓度影响因素比较多,在临床使用过程当中需根据患者的具体情况个体化用药,同时临床药师也需加强对患者的用药教育,增加患者用药依从性。OBJECTIVE We analyzed the results and influencing factors on therapeutic drug monitoring of valproic acid(VPA)in patients of our hospital to provide references for the rational use of the drug in the clinic.METHODS 203 patients who underwent therapeutic drug monitoring of valproic acid were selected from our hospital from 2019 to 2021.The correlation between the ratio of concentration to the dose of valproic acid(Concentration/Dose,“C/D”)and factors such as age,sex,weight,liver function,serum creatinine,albumin,dosage form,and combined use of drugs was investigated by single-factor analysis and multiple linear regression methods.RESULTS The results of the univariate analysis showed that age,sex,weight,dosage forms,whether to use cefoperazone sulbactam,and whether to use dehydrating drugs were all related to the plasma concentration of VPA,while the results of multivariate analysis only retained the two independent factors of sex and dosage forms.CONCLUSION There are many factors that affect the therapeutic drug monitoring of VPA.During clinical practice,it’s necessary to provide individualized medication according to the specific conditions of patients.At the same time,clinical pharmacists should strengthen drug education for patients to increase their drug compliance.
