临床药师参与1例血液透析患者脊柱感染的实践与体会

摘　　要：目的探讨临床药师在血液透析的脊柱感染患者中的个体化药学服务,为临床对该类患者的药学监护提供参考。方法临床药师通过参与1例肾病综合征(需定期透析)合并脊柱感染的患者药物治疗全过程,通过与患者、医生的充分沟通,了解患者病情,并通过查阅指南、文献等相关循证依据,提出个体化治疗方案,协助医生制定抗感染方案,并对患者不良反应进行密切监护。结果临床药师较好融入临床治疗团队,通过密切跟踪患者病情、依托血药浓度监测,患者好转后出院。结论临床药师应结合专科特点,对特定人群进行目标性的药学服务,并且不断提高专业能力和自身的工作能力,提高药学服务的水平。OBJECTIVE To explore the individualized pharmacy service of clinical pharmacists in the spinal-infected patients on hemodialysis,to provide a reference for the clinical pharmacy monitoring of such patients.METHODS Clinical pharmacists participated in the treatment of a patient with nephrotic syndrome(requiring regular dialysis)combined with spinal infection.Through communication with patients,doctors,review the guidelines,documents and other related evidence basis,proposed individual treatment options,participated in formulating anti-infective programmes and monitoring of adverse reactions in patients.RESULTS Clinical pharmacists integrated into the clinical team,tracking the patients with the disease,relying on therapeutic drug monitoring,the patient was discharged.CONCLUSION Clinical pharmacists should combine the characteristics of specialties,provide pharmaceutical care for specific groups,and constantly improve their professional and working ability,in order to improve the level of pharmaceutical care.
