静脉用免疫球蛋白在网织红细胞正常新生儿ABO溶血中的临床应用

摘　　要：目的探讨静脉用免疫球蛋白(Intravenous immunoglobulin,IVIG)在网织红细胞(Reticulocyte,Ret)正常新生儿ABO溶血患儿中的临床应用疗效。方法采用回顾性分析方法,将2018年1月~2020年11月收住于我院NICU并确诊为新生儿ABO溶血的60例患儿纳入本次研究。分为IVIG组(IVIG单次剂量1 g·kg^(-1) ivgtt+光疗)和非IVIG组(仅光疗),各30例。对比两组光疗时间、黄疸消退时间、住院时间以及不良反应。结果60例新生儿ABO溶血患儿Ret正常(Ret<6%),入院日龄约1天到3天。IVIG组光疗时间(41.28±2.91)h、黄疸消退时间(5.41±0.51)d、住院时间(6.72±1.31)d;非IVIG组光疗时间(44.24±4.51)h、黄疸消退时间(5.24±0.44)d、住院时间(6.83±1.26)d。两组均无严重不良反应,未出现胆红素脑病。两组间无统计学差异(P>0.05)。结论IVIG在治疗Ret正常新生儿ABO溶血患儿中非必须用药。OBJECTIVE To explore the clinical efficacy of intravenous immunoglobulin(IVIG)in Neonatal ABO hemolysis with normal reticulocyte(Ret).METHODS Sixty newborns with ABO hemolysis were admitted to NICU in our hospital from January 2018 to November 2020.These newborn patients were divided into IVIG group(IVIG single dose of 1 g·kg^(-1) ivgtt+phototherapy,n=30)and non-IVIG group(phototherapy only,n=30).The phototherapy time,jaundice resolution time,hospital stay and adverse reactions were retrospectively compared between the two groups.RESULTS Sixty newborns had normal Ret(Ret<6%),the age of admission was about 1 to 3 days.In IVIG group,the phototherapy time was(41.28±2.91)h,the jaundice resolution time was(5.41±0.51)d,hospital stay time was(6.72±1.31)d;In the non-IVIG group,the phototherapy time was(44.24±4.51)h,the jaundice resolution time was(5.24±0.44)d,and the hospital stay time was(6.83±1.26)d.There were no serious adverse reactions and no bilirubin encephalopathy in both groups.There was no statistical difference between the two groups(P>0.05).CONCLUSION IVIG is not necessary in the treatment of Neonatal ABO hemolysis with normal Ret.
