电针董氏奇穴联合舒肛安痛方中药坐浴治疗功能性肛门直肠痛的临床研究

摘　　要：目的观察电针董氏奇穴联合舒肛安痛方中药坐浴治疗功能性肛门直肠痛的临床疗效。方法将纳入的120例患者随机分为3组各40例,电针组电针董氏奇穴,中药组舒肛安痛方中药坐浴,联合组电针董氏奇穴并联合舒肛安痛方中药坐浴。比较各组治疗前后视觉模拟量表(VAS)评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、简明汉化版健康调查简表(SF-36)生活质量量表评分、肛门直肠测压、各组患者临床疗效及不良反应。结果联合组的临床总有效率为85.00%,显著高于电针组的62.50%、中药组的42.50%(P<0.05);治疗后各组的VAS评分、SAS评分、SDS评分、肛门直肠测压评分均较治疗前降低(P<0.05),SF-36生活质量量表评分均较治疗前升高(P<0.05),联合组的VAS评分、SAS评分、SDS评分、肛门直肠测压评分显著低于电针组及中药组(P<0.05),SF-36生活质量量表评分高于电针组及中药组(P<0.05);各组研究对象均未发生明显不良反应及相关并发症。结论电针董氏奇穴联合舒肛安痛方中药坐浴治疗功能性肛门直肠痛临床效果显著,可有效减轻肛门直肠部位的疼痛,缓解盆底肌群异常痉挛及收缩状态,且对敏感、焦虑、抑郁、烦躁不安等不良情绪能起到明显改善作用。Objective:To observe the clinical effect of electroacupuncture at Dong′ s unique point combined with Shugang Antong Decoction in the treatment of functional anorectal pain. Methods:A total of 120 patients were selected and randomly divided into the electroacupuncture group(40 cases),traditional Chinese medicine group(40 cases)and combination group(40 cases). The electroacupuncture group adopted electroacupuncture at Dong′s unique point. The traditional Chinese medicine group was treated with traditional Chinese medicine hip bath of Shugang Antong Decoction,and the combined group was treated with electroacupuncture at Dong′s unique point combined with traditional Chinese medicine hip bath of Shugang Antong Decoction. The VAS score,SAS score,SDS score,SF-36 quality of life scale score,anorectal manometry,clinical efficacy and adverse reactions of the three groups of patients before and after treatment were compared. Results:The total clinical effective rate of the combined group was 85.00%,which was significantly higher than 62.50% in the electroacupuncture group(P <0.05)and 42.50% in the traditional Chinese medicine group(P < 0.05);after treatment,the VAS score,SAS score,SDS score and anorectal manometry score of the three groups were lower than those before treatment(P < 0.05),and the SF-36 quality of life scale score was higher than that before treatment(P < 0.05). After treatment,the VAS score,SAS score,SDS score and anorectal manometry score of the combined group were significantly lower than those of the electroacupuncture group and the traditional Chinese medicine group(P < 0.05),and the SF-36quality of life scale score was higher than those of the electroacupuncture group and the traditional Chinese medicine group(P < 0.05).
