自拟消痛散外敷联合艾瑞昔布治疗急性痛风性关节炎临床观察

摘　　要：目的 探讨自拟消痛散外敷联合艾瑞昔布治疗急性痛风性关节炎的临床价值。方法 选择急性痛风性关节炎患者60例,按随机数字表法分为两组各30例,研究组患者采用自拟消痛散外敷联合艾瑞昔布,对照组患者服用艾瑞昔布,对两组服药后的总有效率进行测评,用药前后调查两组症状积分,测定两组用药前后的血尿酸(UA)、血沉(ESR)、C反应蛋白(CRP)、环氧合酶-2(COX-2)、白细胞介素-1β(IL-1β)、转化生长因子-β(TGF-β)、炎性小体(NALP3)、人8羟基脱氧鸟苷(8-OHdG)、人3-硝基酪氨酸(3-NT),统计两组不良反应(皮肤过敏、瘙痒、皮肤破溃),用药前、用药后1、3、7 d评价两组疼痛视觉模拟量表(VAS)评分,比较两组治疗结果。结果 研究组总有效率为90.00%,高于对照组的63.33%(P <0.05)。用药后研究组热痛、红肿、功能障碍积分均低于对照组(P <0.05)。研究组不良反应10.00%与对照组16.67%比较差异无统计学意义(P> 0.05)。用药后研究组UA、ESR、CRP、COX-2、IL-1β、TGF-β、NALP3、8-OHdG、3-NT水平均低于对照组(P <0.05)。用药后1、3、7 d研究组VAS评分低于对照组(P <0.05)。结论 自拟消痛散外敷联合艾瑞昔布治疗效果更为显著,能够缓解炎性反应,促进红肿疼痛症状减轻,并调节氧化应激反应,促进关节功能障碍改善。Objective:To study the clinical value of self-made Xiaotong Powder combined with imrecoxib in treating acute gouty arthritis. Methods:A total of 60 acute gouty arthritis patients treated in our hospital were selected. According to random number table,patients were divided into study group(n = 30,external application of self-made Xiaotong Powder and imrecoxib)and control group(n = 30,imrecoxib). Medication outcomes were evaluated through the total effective rate,clinical symptom scores,blood uric acid(UA),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),cyclooxygenase-2(COX-2),interleukin-1β(IL-1β),transforming growth factor-β(TGF-β),inflammasome NALP3,human 8-hydroxy-deoxyguanosine(8-OHdG),human 3-nitrotyrosine(3-NT),adverse reactions like skin allergy,pruritus and skin ulceration as well as visual analogue scale(VAS)scores before medication and after 1,3 and 7 d of medication. Results:The total effective rate in the study group(90.00%)was higher than that of control group(63.33%)(P < 0.05);before medication,the clinical symptom scores between groups were not significantly different(P > 0.05);after medication,the heat pain,swelling and dysfunction scores in the study group were lower than those of control group(P < 0.05);the adverse event rate in the study group(10.00%)and control group(16.67%)was not significantly different(P > 0.05);before medication,serum biochemical indicators between groups were not significantly different(P > 0.05);after medication,the UA,ESR,CRP values in the study group were lower than those of control group(P < 0.05);before medication,the inflammatory factors between groups were not significantly different(P > 0.05);after medication,the COX-2,IL-1β,TGF-β,8-OHdG and3-NT contents in the study group were lower than those of control group(P < 0.05);after 1,3 and 7 d of medication,VAS in the study group were lower than those of control group(P < 0.05).
