尿微量蛋白测定在肾脏疾病中的应用价值

摘　　要：目的:明确尿微量白蛋白(U-ALB)、尿转铁蛋白(TRU)、尿免疫球蛋白(TGU)、尿al微球蛋白(AIM)4种微量蛋白测定在肾脏疾病中的应用价值。方法:纳入医院内科门诊和住院经过确诊为肾脏疾病的77例患者进行实验,利用散射免疫比法分为肾小球肾炎组(35例)、肾盂肾炎组(23例)、糖尿病肾病组(19例),20例健康人群作为对照组,取新鲜中段尿进行以上4种蛋白的测定。结果:肾小球肾炎组U-ALB比对照组更高(P<0.05),其他3项均值比对照组略高,差异无统计学意义(P>0.05);肾盂肾炎组、糖尿病肾病组3项检测结果均高于对照组,而肾盂肾炎组U-ALB和对照组对比,差异无统计学意义(P>0.05);糖尿病肾病组AIM和对照组相比较,差异无统计学意义(P>0.05)。结论:采用以上4种微量蛋白测定定位肾小球和肾小管受损的状况有一定的作用,值得临床推广。Objective:To investigate the application value of urinary microalbumin(U-ALB),urinary transferrin(TRU),urinary immunoglobulin(TGU)and urinary Al microglobulin(AIM)in renal diseases.Methods:in hospital internal medicine outpatient and hospitalization after 77 patients diagnosed with kidney disease,the scattering immune ratio method respectively on 35 cases of 23 cases of glomerulonephritis,pyelonephritis,19 cases of diabetic nephropathy patients and 20 healthy people as control group,take fresh midway through the urine to determine the above four kinds of protein.Results:The U-ALB of glomerulonephritis group was higher than that of control group(P<0.05),and the mean values of other 3 items were higher than that of control group,the difference was not statistically significant(P>0.05).The results of 3 tests in pyelonephritis group and diabetic nephropathy group were higher than those in control group(P<0.05),while the difference of U-AlB between pyelonephritis group and control group was not statistically significant(P>0.05).There was no statistical significance in AIM between diabetic nephropathy group and control group(P>0.05).Conclusion:Using the above four kinds of microproteins to determine and locate the condition of glomerular and renal tubule damage has a certain effect,and is worth promoting in clinical practice.
