不同频率探测音声导抗和声反射在0-12个月婴幼儿中的应用

摘　　要：目的探讨0-12个月正常婴幼儿中226Hz和1KHz探测音声导抗,声反射(acoustic reflex,AR),声反射阈(acoustic stapedial reflex threshold,ASRT)的特点。方法选取符合纳入条件的0-12个月正常婴幼儿96例(139耳),按照0-3月龄,4-6月龄,7-12月龄分成三组,分别进行226Hz和1KHz声导抗的鼓室图和同侧AR测试,比较三组声导抗,同侧AR引出率和ASRT值。结果①0-12个月龄听力正常婴幼儿226Hz探测音声导抗引出率为100%。②0-12个月龄听力正常婴幼儿1KHz探测音声导抗分型单峰占81.29%,单峰负压占3.6%,双峰占12.23%,异常占2.88%。③0-12个月龄听力正常婴幼儿1KHz探测音下1KHz,2KHz刺激音AR引出率均高于226Hz探测音下1KHz,2KHz刺激音AR引出率。④0-12个月龄听力正常婴幼儿1KHz探测音下1KHz,2KHz刺激音ASRT值均低于226Hz探测音下1KHz,2KHz刺激音ASRT值。结论在0-12个月正常婴幼儿中226Hz探测音声导抗引出率高于1KHz探测音声导抗。0~12个月龄婴幼儿建议选用1KHz探测音行AR测试,对于226Hz和1KHz探测音在不同频率刺激音的ASRT值的范围还需进一步的研究。Objective To investigate the characteristics of tympanogram,acoustic reflex and acoustic stapedial reflex threshold of 226Hz and 1kHz acoustic immittance in normal infants aged 0-12 months.Methods Ninety six normal infants(139 ears)aged 0-12 months were selected and divided into three groups according to the age of 0-3months,4-6 months and 7-12 months.The tympanogram and ipsilateral AR test of 226Hz and 1kHz acoustic immittance were performed respectively.The tympanogram,ipsilateral AR extraction rate and ASRT value of the three groups were compared.Results①The extraction rate of 226Hz acoustic immittance of normal hearing infants aged 0-12 months was 100%.②In the types of 1kHz acoustic immittance in infants aged 0~12 momths with normal hearing,81.29%of them had single peak,3.6%had negative single peak,12.23%had double peak and 2.88%was abnormal.③The AR extraction rate of 1kHz and 2KHz stimulus under 1kHz detection tone was higher than that of 1kHz and 2KHz stimulus under 226hz detection tone in 0-12 months old infants with normal hearing.④The ASRT values of 1kHz and 2KHz stimulus tone under 1kHz detection tone were lower than those of 1kHz and 2KHz stimulus tone under 226hz detection tone in 0-12 months old infants with normal hearing.Conclusion In normal infants aged 0-12 months,the extraction rate of 226Hz acoustic immittance is 100%,and the sensitivity is high.For infants at 0 and 12 months,the AR test with 1000 Hz probe tone showed more advantages over 226 Hz probe tone.The range of ASRT value of 226Hz and 1kHz probe tone at different frequencies needs further study.
