摘 要:目的:通过观察不同体表刺激方式对坐骨神经痛大鼠环跳、阳陵泉穴区局部皮肤组织相关化学物质5-羟色胺(5-HT)、神经肽Y(NPY)、降钙素基因相关肽(CGRP)的影响,初探电针、艾灸、刮痧治疗坐骨神经痛的生物学机制。方法:将35只雄性SD大鼠随机分为空白组、模型组、电针+模型组、艾灸+模型组、刮痧+模型组,每组7只,造模第3天选取大鼠左后肢环跳、阳陵泉穴,分别按组进行电针、艾灸、刮痧干预,以4%甲醛灌流固定,取穴区局部皮肤组织进行组织形态学检测。结果:环跳穴区局部皮肤组织的表皮层、真皮层均有5-HT阳性标记,阳陵泉穴区局部皮肤组织表皮层、真皮层、皮下组织均有5-HT阳性标记,其在表皮下与真皮浅层形成与表皮平行的不连续细胞带,在真皮深层主要分布在毛囊、腺体、血管周围,在皮下组织主要聚集在血管旁。与空白组比较,模型组环跳、阳陵泉穴区5-HT阳性标记均显著升高(P<0.05);与模型组比较,各治疗组环跳、阳陵泉穴区5-HT阳性标记均显著减少(P<0.05)。NPY主要以串珠形式分布在环跳穴区局部皮肤组织的真皮深层绕毛囊或血管分布。与空白组比较,模型组NPY阳性表达纤维显著减少(P<0.05);与模型组比较,各治疗组NPY阳性表达纤维显著增多(P<0.05)。阳陵泉穴区局部皮肤组织中CGRP主要呈串珠样,在真皮浅层接近表皮部位呈现游离状态,真皮层深部及皮下组织绕血管分布,与空白组比较,模型组CGRP阳性表达纤维显著增多(P<0.05);与模型组比较,各治疗组CGRP阳性表达纤维显著减少(P<0.05)。结论:电针、艾灸、刮痧均可使坐骨神经痛大鼠穴区局部皮肤组织中5-HT、NPY、CGRP表达向着正常水平方向逆转,表明5-HT、NPY、CGRP参与了坐骨神经治疗,其可能是针刺、艾灸、刮痧治疗坐骨神经痛的共同物质基础。Objective:To observe the effects of different body surface stimulation methods on the chemical substances related to local skin tissue in Huantiao and Yanglingquan acupoints of sciatica rats to explore the biological mechanism of electroacupuncture,moxibustion and scraping in the treatment of sciatica.Methods:Thirty five male SD rats were randomly divided into blank group,model group,electroacupuncture+model group,moxibustion+model group,scraping+model group,with 7 rats in each group.On the third day of modeling,the left hind limb Huantiao and Yanglingquan acupoints of rats were selected and intervened by electroacupuncture,moxibustion and scraping,then the rats were perfused and fixed with 4%formaldehyde,and the local skin tissue of the acupoint area was taken for histomorphological examination,and the relevant data were analyzed by statistical principle.Results:There were 5-HT positive markers in the epidermis and dermis of local skin tissues in the Huantiao point area,there were 5-HT positive markers in the epidermis,dermis and subcutaneous tissues of local skin tissues in the Yanglingquan point area.They formed discontinuous cell bands parallel to the epidermis under the epidermis and the superficial layer of the dermis.In the deep layer of the dermis,they were mainly distributed around hair follicles,glands and blood vessels.In the subcutaneous tissue,they were mainly gathered near the blood vessels.Compared with the blank group,the 5-HT positive mark in the model group increased significantly(P<0.05);compared with the model group,the positive rate of 5-HT in moxibustion+model group,electroacupuncture+model group and scraping+model group decreased significantly(P<0.05).NPY was mainly distributed around hair follicles or blood vessels in the deep dermis of local skin tissue in the Huantiao point area in the form of beads.Compared with the blank group,the NPY positive fibers in the model group were significantly reduced(P<0.05);compared with the model group,the NPY positive fibers in the intervention groups we
