针刺结合耳穴按压治疗烟草依赖的临床随机对照试验

摘　　要：目的:观察针刺结合耳穴按压治疗烟草依赖的临床疗效。方法:将90例患者采用随机数表法分成针刺结合耳穴组(试验组)、针刺组及耳穴组,每组30例。试验组干预措施为针刺结合耳穴按压;针刺组为单纯针刺,耳穴组为单纯耳穴按压,针刺组及耳穴组取穴均同试验组。均每周治疗2次,8周为1个疗程,治疗1个疗程。结果:治疗后,试验组戒断率为40.0%(12/30),针刺组为30.0%(9/30),耳穴组为33.3%(10/30)。3组烟草依赖患者呼出气体CO含量均较治疗前显著改善(P<0.05),试验组对于呼出CO气体含量减少程度优于针刺组(P<0.05)。3组FTND量表评分较治疗前均显著下降(P<0.05),试验组与针刺组对AUTOS量表总评分治疗前后差值的改善均优于耳穴组(P<0.05)。试验组和针刺组在AUTOS量表3个维度(戒断症状、精神依赖及烟瘾诱因)方面优于耳穴组(P<0.05)。试验组和耳穴组治疗后患者每日吸烟量减少的速度和程度均优于针刺组(P<0.01)。结论:针刺结合耳穴按压对烟草依赖有明确的临床疗效,且综合疗效优于单纯针刺及单纯耳穴按压,单纯针刺能更好控制患者的戒断综合征,耳穴按压能更快更好地减少患者的每日吸烟量。Objective: To observe the clinical effect of acupuncture combined with auricular acupoint pressing on tabacco dependence. Methods: Ninety patients were randomly divided into acupuncture combined with auricular point group(experimental group), acupuncture group, and auricular point group using a random number table method, with 30 cases in each group. The intervention measures of the experimental group were acupuncture combined with auricular point pressing;the acupuncture group was pure acupuncture, and the auricular point group was pure auricular acupoint pressing. The acupuncture group and the auricular point group on the point selection was the same as the experimental group. The three groups were treated twice a week, with a course of 8 weeks for 16 times in total. Results: After treatment, the abstinence rate of experimental group was 40.0%(12/30), that of acupuncture group was 30.0%(9/30), and that of auricular point group was 33.3%(10/30).The CO content in exhaled gas of patients with tobacco dependence in the three groups was significantly improved than that before treatment(P<0.05), and the reduction degree of CO content in the experimental group was better than that in the acupuncture group(P<0.05). The FTND score in the three groups significantly decreased than that before treatment(P<0.05).The improvement of AUTOS score difference before and after treatment in experimental group and acupuncture group was better than that in auricular point group(P<0.05). The experimental group and acupuncture group had improvement in three dimensions of AUTOS(withdrawal symptoms, mental dependence and smoking addiction inducement) than the auricular point group(P<0.05).The speed and degree of daily smoking reduction in experimental group and auricular point group after treatment were better than those in acupuncture group(P<0.01).
