围手术期三步护理联合情志护理对结肠癌患者并发症及营养指标的影响

摘　　要：目的探讨对结肠癌患者联合情志护理与围手术期三步护理的价值,评估其对患者术后并发症发生率和营养指标的影响。方法回顾性分析2019年3月至2022年3月常州市中医医院收治的80例结肠癌患者的基线资料,按照护理措施的不同划分为对照组(41例)和观察组(39例)。对照组采取围手术期三步护理,观察组应用围手术期三步护理联合情志护理干预。对比2组的术后并发症发生率、营养指标、术后恢复状态、手术耐受优良率及负性情绪。结果观察组的术后并发症总发生率较对照组更低,首次肛门排气时间和下床活动时间均较对照组更早,住院时间和术后禁食时间均较对照组更短,手术耐受优良率较对照组更高(P均<0.05)。出院时,2组的机体营养状态均较入院后有所改善(P均<0.05),且观察组均较对照组更优(P均<0.05)。术后1周和出院时,2组的负性情绪评分均较入院后降低(P均<0.05),且观察组均低于同期对照组(P均<0.05)。结论针对结肠癌患者实施围手术期三步护理联合情志护理能够加快患者的术后康复进程,且在预防术后并发症的同时,可改善患者的机体营养状态及心理状态。Objective To explore the value of emotional nursing combined with three-step perioperative nursing in the patients with colon cancer,and to evaluate its effect on the incidence of postoperative complications and nutritional indicators.Methods A retrospective analysis was made on the baseline data of 80 colon cancer patients at Changzhou Traditional Chinese Medicine Hospital from March 2019 to March 2022.They were assigned into a control group(41 cases)and an observation group(39 cases)according to different nursing measures.The control group received three-step perioperative nursing,while the observation group received three-step perioperative nursing combined with emotional nursing intervention.The incidence of postoperative complications,nutritional indicators,postoperative recovery status,excellent and good rate of surgical tolerance,and negative emotions were compared between the two groups.Results The total incidence of postoperative complications in the observation group was lower than that in the control group.The time to first anal exhaust and ambulation,length of hospital stay,and postoperative fasting time were shorter than those in the control group.The excellent and good rate of surgical tolerance was higher than that in the control group(all P<0.05).At discharge,the nutritional status of both groups was improved compared with that at admission(all P<0.05),and the observation group was better than the control group(all P<0.05).At 1 week after operation and at discharge,the scores of negative emotions in both groups were lower than those at admission(all P<0.05),and the scores in the observation group were lower than those in the control group(all P<0.05).Conclusion Three-step perioperative nursing combined with emotional nursing for colon cancer patients can speed up the process of postoperative rehabilitation,prevent the postoperative complications,and improve the nutritional and psychological status.
