自制烫伤宁膏药外敷联合艾灸疗法对Ⅱ期压疮患者愈合效果的影响

摘　　要：目的探讨Ⅱ期压疮患者采用自制烫伤宁膏药外敷联合艾灸疗法干预对其愈合效果的影响。方法以2021年5月至2022年5月广西中医药大学附属瑞康医院收治的84例Ⅱ期压疮患者为研究对象,采用抽签法分为对照组和观察组,每组42例。2组患者均接受临床基础干预,在此基础上,对照组给予艾灸疗法干预,观察组给予自制烫伤宁膏药外敷联合艾灸疗法干预。比较2组的临床指标、压疮愈合情况、炎症反应和创面新生肉芽生长情况。结果干预1个月后,2组的24 h压疮渗液量、创面情况和周边皮肤情况评分均低于干预前(P均<0.05),C反应蛋白(CRP)、白介素-6(IL-6)和白介素-1β(IL-1β)水平均低于干预前(P均<0.05),压疮愈合评分量表(PUSH)和视觉模拟评分法(VAS)评分均低于干预前(P均<0.05),血管内皮生长因子(VEGF)和表皮生长因子(EGF)水平均高于干预前(P均<0.05);观察组在干预1个月后的上述所有指标均较对照组更佳(P均<0.05)。结论采用自制烫伤宁膏药外敷联合艾灸疗法对Ⅱ期压疮患者实施干预,可改善其临床指标,促进创面愈合和新生肉芽组织生长,减轻炎症反应。Objective To explore the effect of topical application of self-made burning plaster combined with moxibustion therapy on the healing of patients with stage 2 pressure ulcers.Methods A total of 84 patients with stage 2 pressure ulcers at Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine from May 2021 to May 2022 were selected as the research subjects and divided into a control group and an observation group by lottery method,with 42 cases in each group.Both groups received basic clinical intervention.The control group was additionally treated with moxibustion therapy,while the observation group was additionally treated with topical application of self-made burning plaster combined with moxibustion therapy.The clinical indicators,pressure ulcer healing,inflammatory response,and granulation tissue forming of wounds were compared between the two groups.Results After 1 month of intervention,the scores of 24-hr pressure ulcer exudate volume,wound status,and peripheral skin status in the two groups were lower than those before intervention(all P<0.05).The levels of C-reactive protein(CRP),interleukin-6(IL-6),and interleukin-1β(IL-1β)were lower than those before intervention(all P<0.05).The scores of Pressure Ulcer Scale for Healing(PUSH)and Visual Analogue Scale(VAS)were lower than those before intervention(all P<0.05).The levels of vascular endothelial growth factor(VEGF)and epidermal growth factor(EGF)were higher than those before intervention(all P<0.05).All the above indicators in the observation group were better than those in the control group after 1 month of intervention(all P<0.05).Conclusion Topical application of self-made burning plaster combined with moxibustion therapy for patients with stage 2 pressure ulcers can improve the clinical indicators,promote the wound healing and granulation tissue forming,and reduce the inflammatory response.
