合肽素、同型半胱氨酸及心肌型脂肪酸结合蛋白检测在急性肺栓塞病情评估中的应用分析

摘　　要：目的 分析合肽素、同型半胱氨酸(Hcy)及心肌型脂肪酸结合蛋白(H-FABP)检测在急性肺栓塞(APE)病情评估中的应用。方法 选取2016年2月至2022年2月我院收治的112例APE患者为观察组,按病情程度分为高、中、低危组分别为20例、41例和51例。另收集同期健康体检者118例为对照组。对比不同人群合肽素、Hcy、H-FABP水平及APACHEⅡ评分;分析合肽素、Hcy及H-FABP与APACHEⅡ评分的相关性。结果 观察组合肽素、Hcy及H-FABP高于对照组(P<0.05);高危组合肽素、Hcy、H-FABP、APACHEⅡ高于低中危组(P<0.05);APE患者血清合肽素、Hcy及H-FABP与APACHEⅡ评分均为正相关(P<0.05)。结论 合肽素、Hcy及H-FABP可作为APE病情评估指标,其水平随着病情程度增加而上升,且对于临床治疗具有一定指导意义。Objective To analyze the application of the detection of copeptin, homocysteine(Hcy) and myocardial fatty acid binding protein(H-FABP) in the evaluation of acute pulmonary embolism(APE).Methods A total of 112 patients with APE admitted in our hospital from February 2016 to February 2022 were selected as an observation group. The patients were divided into a high risk group(n=20), a medium risk group(n=41) and a low risk group(n=51) according to the degree of disease. In addition, 118 healthy subjects were collected as a control group. Levels of copeptin, Hcy and H-FABP and Apache Ⅱ score in different populations were compared. The correlation between the copeptin, Hcy, H-FABP and the Apache Ⅱ score was analyzed.Results The levels of copeptin, Hcy and H-FABP in the observation group were higher than those in the control group(P<0.05). Copeptin, Hcy, H-FABP and Apache Ⅱ in the high-risk group were higher than those in the low and medium risk groups(P<0.05). Serum copeptin, Hcy and H-FABP were positively correlated with Apache Ⅱ score in patients with APE(P <0.05).Conclusions Copeptin, Hcy and H-FABP can be used as indicators for evaluating the disease changes of APE. Their levels rise with the increase of the disease degree. They have certain guiding significance for clinical treatment.
