核磁共振成像检查子宫内膜癌患者的诊断价值

摘　　要：目的探讨核磁共振成像检查子宫内膜癌患者的诊断价值。方法选取55例疑似子宫内膜癌患者,均实施核磁共振成像检查,以手术病理结果为金标准,分析核磁共振成像的诊断效能及术前分期诊断结果与病理检查的一致性。结果核磁共振成像对子宫内膜癌诊断准确率、敏感性、特异性分别为98.18%、100%、87.50%。核磁共振成像检查对子宫内膜癌分期诊断准确率82.98%(39/47),对Ⅰa、Ⅰb、Ⅱa、Ⅱb、Ⅲa、Ⅲb、Ⅲc诊断准确率83.33%(10/12)、76.92%(10/13)、77.78%(7/9)、83.33%(5/6).100%(4/4)、100%(2/2)、100%(1/1),与金标准比较一致性较好(Kappa=0.79),差异无统计学意义(χ^(2)=2.667,P=0.615>0.05)。结论核磁共振成像检查对子宫内膜癌的诊断效能较高,且术前分期诊断与金标准一致性较好,可为临床诊疗提供可靠依据。Objective To investigate the diagnostic value of MRI in patients with endometrial carcinoma. Methods A total of 55 patients with suspected endometrial cancer were selected for MRI examination. The diagnostic efficacy of MRI and the consistency of preoperative staging diagnosis results with pathological examination were analyzed with surgical pathology results as the gold standard. Results The diagnostic accuracy, sensitivity and specificity of MRI for endometrial cancer were 98.18%,100% and 87.50%, respectively. The accuracy of MRI in staging diagnosis of endometrial cancer is 82.98%(39/47). The diagnostic accuracy of Ⅰa, Ⅰb, Ⅱa, Ⅱb, Ⅲa, Ⅲb, Ⅲc was 83.33%(10/12), 76.92%(10/13), 77.78%(7/9), 83.33%(5/6), 100%(4/4), 100%(2/2), 100%(1/1). Compared with the gold standard, there was a good consistency(Kappa=0.79), with no statistical significance(χ^(2)=2.667, P=0.615>0.05). Conclusion Magnetic resonance imaging has a high diagnostic efficiency for endometrial cancer, and the preoperative staging is in good agreement with the gold standard, which can provide a reliable basis for clinical diagnosis and treatment.
