摘  要:目的采用横断面研究方法分析人群血尿酸与血压的关系,为双重危险因素防治提供科学依据。方法采用年龄分层(20~79岁)、随机抽选连续5年(2015—2019年)上海交通大学附属第六人民医院体检中心600名体检者,男女各半。采集该人群的病史、身高、体重、腰围、血压、血液生化指标,其中资料完整且符合纳入标准者524人(87.3%),体检项目与相关参数均采用统一标准化方法。研究对象根据血尿酸水平的四分位数分组:第一分位组(血尿酸141.0~<260.5μmol/L,n=131)、第二分位组(血尿酸260.5~<315.0μmol/L,n=132)、第三分位组(血尿酸315.0~<375.0μmol/L,n=131)、第四分位组(血尿酸375.0~683.0μmol/L,n=130)。分析高血压与血尿酸关联的密切程度。结果在血尿酸四分位数分组中存在性别分布差异,高血尿酸值区(第四分位组)男性所占的比例显著高于女性,而女性主要集中于低血尿酸值区(第一分位组);研究人群的血尿酸与收缩压、舒张压均存在线性关系(P值分别为0.0018和0.0005),这种差异仅表现在女性。结论血尿酸与高血压显著相关,青年男性和绝经后女性应多关注自身的血尿酸水平。Objective A cross-sectional study method was used to analyze the relationship between serum uric acid and blood pressure in the population,and provide scientific basis for the prevention and treatment of dual risk factors.Methods Using age stratification(20-79 years old),600 medical examiners in the Shanghai Jiao Tong University Affliated Sixth People’s Hospital Medical Examination Center,half of men and women,were randomly selected for 5 consecutive years(2015-2019).The medical history,height,weight,waist circumference,blood pressure,and blood biochemical indexes of this population were collected.Among them,524 people(87.3%)had complete data and met the inclusion criteria.The physical examination items and related parameters were uniformly standardized.The people were divided into 4 groups according to serum uric acid level:the first quantile group(serum uric acid 141.0-<260.5μmol/L,n=131),the second quantile group(serum uric acid 260.5-<315.0μmol/L,n=132),the third quantile group(serum uric acid 315.0-<375.0μmol/L,n=131),the fourth quantile group(serum uric acid 375.0-683.0μmol/L,n=130)to analyze the close degree of the relationship between hypertension and serum uric acid.Results There are sex distribution differences in the serum uric acid quartile groupings.The proportion of men in the high serum uric acid area(the fourth quartile group)is significantly higher than that of women,while women are mainly concentrated in the low serum uric acid area(the first quantile group).There is a linear relationship between serum uric acid and systolic blood pressure and diastolic blood pressure in the study population(P value are 0.0018 and 0.0005,respectively),and this difference is only manifested in women.Conclusion Serum uric acid is significantly related to high blood pressure.Young men and postmenopausal women should pay more attention to their serum uric acid levels.
