摘  要:目的探索基于图像存储与传输系统(picture archiving and communication system,PACS)的以案例里为基础的教学法(case-based learning,CBL)在胃镜操作学习中的作用。方法选取2018年2月—2021年2月在武汉大学人民医院消化内科学习胃镜操作的学生,按信封法随机分成对照组和试验组。对照组采用理论授课结合实践操作教学法,试验组采用基于PACS的CBL教学方法。分别在胃镜操作第一阶段(0~10例)、第二阶段(11~20例)及第三阶段(21~30例)时对两组学生进行操作及诊断能力测试,最后进行教学方法的问卷调查及结果分析。结果试验组胃镜操作评分在第二、三阶段(11~30例)要优于对照组,差异有统计学意义(P<0.05);诊断准确性教学效果比较中,试验组均优于对照组,差异有统计学意义(P<0.05)。问卷调查显示试验组对于该新型教学模式满意度高于对照组的传统教学模式。结论采用基于PACS的CBL教学能够激发学生自主学习兴趣,较快提高胃镜操作能力及诊断准确性。Objective To explore the role of casebased learning(CBL)based on picture archiving and communication system(PACS)in gastroscope learning.Methods The students who had been learning gastroscopy at Renmin hospital of Wuhan University between Feb 2018-Feb 2021,were randomly divided into control group and experimental group according to envelope method.The control group adopted theory teaching combined with practice.CBL method based on PACS was used in the experimental group.After the first stage of gastroscopy learning(0-10 cases),the second stage(11-20 cases)and the third stage(21-30 cases),students in these two groups were tested for their operational and diagnostic skill.Finally,a questionnaire survey was conducted on the students and the results were analyzed.Results The score of gastroscope operation in the test group was better than that in the control group in the second and third stages(11-30 cases),with a statistically significant difference(P<0.05).In the comparison of the teaching effect of diagnostic accuracy,the experimental group was better than the control group,and the difference was statistically significant(P<0.05).The questionnaire survey showed that the experimental group was more satisfied with the new teaching mode than the control group with the traditional teaching mode.Conclusion CBL teaching based on PACS inspires self-directed learning,improves students’operation ability and diagnostic accuracy of gastroscope.It has generalizable value compared with traditional teaching method.
