影像导航在鼻内镜手术住培中的应用探讨

摘　　要：鼻内镜手术是住院医师规范化培训的重点培训内容之一,但由于鼻-鼻窦解剖结构复杂,同时毗邻颅底、视神经等重要解剖部位,兼之鼻内镜视野灵活,缺乏层次感,一直以来是住培教学的难点。影像导航鼻内镜可以将影像与鼻内镜图像实时匹配、定位,使得鼻内镜手术不再是“摸黑前进”。针对住培医师学习时间短、学习任务重的特点,将影像导航鼻内镜系统引入耳鼻咽喉科住院医师规范化培训的鼻内镜手术教学中,通过理论讲授、鼻部影像阅片、影像导航内镜解剖教学、鼻内镜操作基本功训练与影像导航辅助的鼻内镜手术操作训练相结合的教学方式,既改善了传统鼻内镜手术教学不直观的问题,缩短了学习曲线,也增强了住培医师鼻内镜操作的自信心,为日后的鼻内镜手术操作打下了扎实基础。该方法在耳鼻咽喉科住培教学中能有效提高教学质量。Objective Nasal endoscopic surgery is one of the key training of standardized training for residents.However,it is difficult of teaching due to the large anatomical variation of the sinuses.The image-guided nasal endoscopy can match and locate the image and the nasal endoscopic image in real time,so that the nasal endoscopic surgery is no longer"in the darkness".Therefore,we introduced the image-guided nasal endoscopy system into the nasal endoscopic surgery teaching.The result shows that image-guided endoscopy is an effective technique for nasal endoscopic surgery teaching.The improved method is worthy of being promoted in the teaching of standardized training for residents.
