不同年龄语前聋儿童人工耳蜗植入术效果分析

摘　　要：目的探讨不同年龄的语前聋儿童人工耳蜗植入术后康复效果及其对生活质量的影响。方法回顾性选取2015年6月至2020年5月河南省南阳市中心医院收治的语前聋儿童120例,按照儿童年龄将其分为低龄组(<3岁,37例)、中龄组(3~7岁,58例)和大龄组(>7岁,25例)。所有儿童均接受人工耳蜗植入术;术后6个月评估听觉行为分级、有意义听觉整合量表、语言交流能力、言语可懂度分级;术后1年评估Nijmegen人工耳蜗植入量表、生活质量。结果术后6个月,听觉行为分级、有意义听觉整合量表得分率、轮流交流能力(V+G)、主动交流能力(V+G)、言语可懂度分级各项分数、术后1年的基本声音感知、高级声音感知、言语能力、活动能力、社会交流、自信心各项分数从高到低排序为低龄组、中龄组、大龄组;与术前比,术后1年三组父母期望、对家庭影响、生活质量各项评分均升高,且从高到低排序为低龄组、中龄组、大龄组(P<0.05)。结论不同年龄语前聋儿童接受人工耳蜗植入术均可促进听觉、言语发育,且术后1年内儿童年龄越小效果越显著,在轮流交流能力、主动交流能力、言语可懂度分级及生存质量方面的提升效果较好。Objective To investigate the postoperative rehabilitation effect of cochlear implantation in prelingually deaf children of different ages and its impact on quality of life.Method A total of 120 children with prelingual deafness admitted to our hospital from June 2015 to May 2020 were retrospectively selected,and they were divided into a younger age group(<3 years old,37 cases)and a middle age group(3~7 years old,58 cases)and older age group(>7 years old,25 cases)according to their age.All children received cochlear implantation;Auditory behavior scale,meaningful Auditory Integration Scale,verbal communication ability and speech intelligibility were evaluated six months after surgery.Nijmegen cochlear implant scale and quality of life were evaluated 1 year after surgery.Result Six months after surgery,Auditory behavior grading,score rate of meaningful Auditory Integration Scale,alternating communication ability(V+G),active communication ability(V+G),scores of speech intelligibility grading,and scores of basic voice perception,advanced voice perception,speech ability,activity ability,social communication and confidence one year after surgery were ranked from high to low was the younger age group,the middle age group and the older age group.Compared with pre-operation,scores of parental expectation,influence on family and quality of life in the three groups were all increased one year after surgery,and the ranking from high to low was the younger age group,the middle age group and the older age group(P<0.05).Conclusion Cochlear implantation could promote the development of hearing and speech in children with prelingual deafness of different ages.In addition,within 1 year after surgery,the younger the children,the more significant the effect,which could improve the ability of alternating communication,the ability of active communication,the grade of verbal intelligibility and the quality of life.
