摘  要:中医药在肝纤维化防治中不断有新的突破,越来越多的研究证实了中医药在肝纤维化的治疗方面具有独特优势。目前,中医药抗纤维化的主要机制有以下几方面:抑制肝星状细胞(hepatic stellate cells,HSC)增殖活化,减少肝细胞损伤;促进HSC凋亡;降低炎症因子水平,减轻肝脏炎症反应;抑制细胞外基质(extracellular matrix,ECM)的合成、降解;降低肝窦阻力,抑制肝窦毛细血管化;调节胶原代谢,降低肝组织胶原纤维水平;调控信号通路,影响细胞内信号传导;改善肝脏代谢;减轻氧化应激;降低蛋白表达,调节免疫应答等。通过总结近3年来中医药治疗肝纤维化的分子机制,为临床应用中医药逆转肝纤维及抑制肝纤维化进展方面提供思路。Traditional Chinese medicine has made new breakthroughs in the prevention and treatment of liver fibrosis.More and more studies have confirmed that traditional Chinese medicine has unique advantages in the treatment of liver fibrosis.At present,the main anti-fibrosis mechanisms of TCM are as follows:inhibiting the proliferation and activation of hepatic stellate cell(HSC)and reduce the damage of hepatocytes;promoting HSC apoptosis,reducing the levels of inflammatory factors,reducing liver inflammation,inhibiting the synthesis and degradation of extracellular matrix(ECM).It can reduce the resistance of hepatic sinuses and inhibit the capillarization of hepatic sinuses,regulate collagen metabolism and reduce the level of collagen fiber in liver tissue,regulate signal pathway,affect intracellular signal transduction,improve liver metabolism,reduce oxidative stress,decrease protein expression and regulate immune response.By summarizing the molecular mechanism of TCM in the treatment of liver fibrosis in recent 3 years,it provides ideas for the clinical application of TCM to reverse liver fibrosis and inhibit the progression of liver fibrosis.
